Objectives:Based on the theory of five evolutive phases and six climatic factors,by collecting the basic data of patients with pulmonary nodules, the characteristics of five evolutive phases and six climatic factors on the date of birth of patients with pulmonary nodules were summarized. At the same time, the patients were classified by TCM constitution, and the correlation between the characteristics of five evolutive phases and six climatic factors on the date of birth of patients with pulmonary nodules and TCM constitution was explored, which provided new ideas for early prevention and treatment of pulmonary nodules. Methods:In this study,360 cases of pulmonary nodules were collected from January 2022 to January 2023 in the Department of Pulmonary Disease,Affiliated Hospital of Shandong University of Traditional Chinese Medicine. The basic information was collected, and the corresponding data of five evolutive phases and six climatic factors were deduced according to the collected date of birth. At the same time, according to the ’ TCM constitution scale’,the constitution of patients with nodules was classified and determined, and SPSS26.0 was used for statistical analysis to explore the characteristics of five evolutive phases and six climatic factors, the distribution of TCM constitution and their correlation in patients with pulmonary nodules at birth.Results:1.General date:360 patients, including 147 males(40.8%)and 213 females(59.2%);The age was concentrated in 51-61 years old, with the minimum age being 19 years old and the maximum age being 75 years old.In terms of nodule size, the maximum nodule diameter was mainly 5-10mm.In terms of the number of nodules,151 patients(41.9%)had single nodules, and 209 patients(58.1%)had multiple nodules. There were 73 patients(20.3%)with definite personal history family history of tumor. All 360 patients had a history of exposure to dust and oil smoke,of which 194 patients were exposed to dust and oil smoke every day.2.Five evolutive phases and six climatic factors:50 cases of lung nodule patients were born in the year of birth, with the highest number of 50 cases in the year of birth, and the least number of 17 cases in the year of birth. The distribution difference was statistically significant(P<0.01).There were 50 cases with less fire and 17 cases with less water in the distribution of birth date, and the difference in the distribution of birth date was statistically significant(P<0.01).On the date of birth, there were 47 cases of Ethereum and 28 cases of Taitu, with no significant difference(P>0.05).On the date of birth, there were 47 cases of less soil distribution and 25 cases of too fire distribution, and there was no significant difference in distribution(P>0.05).There were 66 cases of Shaoyin Junhuo and 50 cases of Taiyin Shitu in the distribution of the main qi on the date of birth, and the difference was not statistically significant(P>0.05).On the date of birth, the distribution of solar cold water was the highest in 82 cases, and the distribution of Shaoyin monarch fire was the least in 51 cases, with statistically significant difference(P<0.05).The characteristics of the birth date of patients with pulmonary nodules were 124 cases born in peace years and 236 cases born in uneven years, and the distribution difference was statistically significant(P<0.01).3.The distribution of constitution:360 cases of pulmonary nodules in patients with TCM constitution from high to low was 105 cases(29.2%)of qi deficiency>82 cases(22.8%)qi stagnation>52 cases(14.4%)of phlegm dampness>40 cases(11.1%)of yin deficiency>30 cases(8.3%)of blood stasis>24 cases(6.7%)of damp heat>16 cases(4.4%)of yang deficiency>7 cases(1.9%)of gentleness constitution>4 cases(1.1%)of special constitution, pulmonary nodules in patients with physical types in the distribution of statistically significant(P<0.01).4.The correlation between five evolutive phases and six climatic factors and constitution:There were statistically significant differences in the distribution of lung nodules in patients with dry birth time, too little age, main qi,guest qi,Sitianquan and constitution(P<0.01).There was a correlation between yin deficiency and the distribution of Sitian in the spring(P<0.05).Yangming dry gold Sitian-Shaoyin Junhuo in the spring has an effect on the formation of yin deficiency, and Yangming dry gold Sitian-Shaoyin Junhuo in the spring is 6.500 times that of Shaoyin Junhuo Sitian-Yangming dry gold in the spring(P<0.05).Jueyin Fengmu Sitian-Shaoyang Xianghuo in Quan has an effect on the formation of phlegm-dampness constitution, and Jueyin Fengmu Sitian-Shaoyang Xianghuo in Quan is 9.735 times that of Shaoyin Junhuo Sitian-Yangming Zaojin in Quan(P<0.05).Conclusion:1.The factors of five evolutive phases and six climatic factors in the date of birth of patients with pulmonary nodules were related to the onset of the disease. The factors of five evolutive phases and six climatic factors were as follows:heavenly stems in Gui year, year evolutive in Shao Huo,guest climatic qi in Tai Yang Hai Shui, and the combination of yun and qi is the uneven year(P<0.05).Host evolutive phases, guest evolutive phases, host climatic qi,Sitian-Zaiquan was examained as P>0.05,it was not statistically significant and cannot be considered to be related to the pathogenesis of pulmonary nodules.2.The occurrence of patients with lung nodules is associated with physical distribution. The top four of the physical distribution are:qi deficiency,qi stagnation, phlegm dampness, yin deficiency.3.The date of birth of patients with pulmonary nodules is related to the formation of acquired constitution, such as heavenly stems, year evolutive, host climatic qi,guest climatic qi and Sitian-Zaiquan.Birth date Sitian-Zaiquan is Yang Ming Zao Jin-Shao Yin Jun Huo is the influencing factor of Yin deficiency constitution in patients with pulmonary nodules.Birth date Sitian-Zaiquan is Jueyin Fengmu-Shao yang Xianghuo is the influencing factor of damp heat constitution in patients with pulmonary nodules.
