ObjectiveBased on evidence-based medicine,this thesis reviews and analyzes relevant guidelines and literature,and constructs a service plan for discharge preparation of patients with atrial fibrillation,so as to promote the smooth transition of patients with atrial fibrillation after discharge and the scientific and standardized self-management of patients outside the hospital.MethodsSet up a research group to search the best practice information book,recommended practices,guidelines,evidence summary,systematic evaluation,expert consensus,etc.of discharge preparation services for patients with atrial fibrillation.For the literature of different types of studies,the literature quality evaluation standard and evidence grading system of evidence-based health care center of Joanna Briggs Institute in Australia were used to evaluate the quality and evidence level,and the first draft of the service plan for atrial fibrillation patients’ discharge preparation was formed on the basis of evidence.The contents of the first draft were revised by Delphi method and modified according to expert opinions to form the final discharge preparation service plan for patients with atrial fibrillation.Results1.This study includes 3 guidelines,3 expert consensus and 4 systematic evaluation.After the evaluation and analysis of the included literature,the first draft of the service plan for the discharge preparation of patients with atrial fibrillation was formed,which was summarized into five dimensions: service purpose,service principle,evaluation,in-hospital service and out-of-hospital service.2.In the revision part of this study,15 clinical experts in the field of cardiology were invited to evaluate the importance and operability of the items of the protocol and put forward specific modification suggestions after two expert letters.The authority coefficient of experts is 0.875,indicating that the selected experts have high authority in the field of clinical care and discharge preparation for atrial fibrillation.After two rounds of expert correspondence,the importance coordination coefficients of experts for the first level items and specific items are 0.149 and 0.213 respectively,and the operability coordination coefficients are 0.379 and 0.376 respectively.The expert opinions are relatively consistent.According to the specific suggestions of the experts,the final draft of the service plan for the discharge preparation of patients with atrial fibrillation with 5 first-level items and 24 specific items has been formed.Conclusions1.This study systematically searched the literature related to the discharge preparation service of patients with atrial fibrillation,and found that there is no structured and effective discharge preparation service system in Chinese mainland at this stage.How medical staff comprehensively evaluate the discharge preparation of patients and their main caregivers,formulate effective personalized interventions,and build a perfect discharge preparation service system will be the focus of our future research.This study evaluates the quality of relevant literature and analyzes the high-quality literature.This study extracted 24 discharge preparation service items from 5 aspects,filling the research gap in the field of discharge preparation services for patients with atrial fibrillation.2.The scheme constructed in this study is formulated based on evidence-based nursing through literature analysis and two rounds of expert correspondence.The experts consulted by correspondence are highly motivated and authoritative.The contents of the constructed scheme are highly scientific and operable,providing a scientific basis for the discharge preparation service of patients with atrial fibrillation,and improving the quality of specialized nursing services and the efficiency of clinical nursing services.
