ObjectiveTo investigate the status quo of family caregiver readiness and its influencing factors for breast cancer surgery patients,and analyze the relationship between caregiver readiness and hope,disease uncertainty and anxiety.To construct the education program of family caregiver role preparation for breast cancer surgery patients,in order to provide reference for clinical improvement of caregiver readiness,caregiver care level and patients’ quality of life.Methods1.The first part is questionnaire survey method: convenience sampling method is adopted to select the family caregivers of breast cancer surgery patients in two Grade A hospitals in Jinan City of Shandong Province.The survey tools included the self-developed general information questionnaire for patients and caregivers,the caregiver Readiness Scale(CPS),the Family Uncertainty Scale(MUIS-MF),the Herth Hope Scale(HHI),and the Zung Self-Rating Anxiety Scale(SAS).To investigate the status quo of care readiness of family caregivers for breast cancer patients undergoing surgery and its relationship with the level of hope,anxiety and disease uncertainty.2.The second part is program construction: On the basis of the previous status investigation,combined with literature review and group discussion,the draft of the education program for the role preparation of family caregivers for breast cancer surgery patients was gradually drawn up.The Delphi expert letter consultation method was adopted to send the "An enabling based educational program for preparing family caregivers for breast cancer surgery patients" after obtaining expert consent.The scheme was modified according to the mean value assigned by experts to the importance of indicators,coefficient of variation and expert suggestions,and the final draft was formed.ResultsA total of 260 questionnaires were sent out in this survey,and 240 were effectively received with an effective recovery rate of 92.3%.The specific results are as follows:1.Results analysis showed that the readiness score of family caregivers for breast cancer surgery patients was(24.53±3.70)points,which was at a medium level,indicating that they were not ready to take care of patients.All items were scored(3.07±0.46)points,among which the physiological need score was the highest(3.86±0.49)points.The lowest score was(2.36±0.60)for emergency response and management.2.The score of family caregivers of breast cancer surgery patients was(91.62±13.34)points,85% of caregivers had a high sense of disease uncertainty,and the entries were all scored(3.05±0.44)points.The score of unpredictability was the highest(3.63±0.58),and the score of lack of information was the lowest(2.31±0.54).3.The hope level of family caregivers of breast cancer surgery patients was higher,the score was(35.04±4.91),and the average score of entries was(2.92±0.41).The highest score was(3.01±0.45)for keeping close contact with others,and the lowest score was(2.88±0.47)for taking positive actions.4.The overall anxiety level of family caregivers of breast cancer surgery patients was low,and the score was(42.22±10.85)points,among which the patients with mild anxiety accounted for 7.5%,the patients with middle anxiety accounted for 8.3%,and the patients with severe anxiety accounted for 2.92%.5.Correlation analysis showed that the readiness of family caregivers for breast cancer surgery patients was negatively correlated with the level of family uncertainty and anxiety,and positively correlated with the level of hope.Disease uncertainty was positively correlated with anxiety level and negatively correlated with hope level.The level of hope is inversely related to the level of anxiety.6.Multiple linear regression analysis showed that factors affecting the readiness of family caregivers for breast cancer surgery patients included caregiver age,education,relationship with patients,whether they lived with each other,monthly family income,caring experience,anxiety,disease uncertainty and hope level,and the above 9 variables could affect 73.7% of the readiness of family caregivers for breast cancer surgery patients.7.Results of expert letter consultation: After two rounds of expert letter consultation,An enabling based educational program for preparing family caregivers for breast cancer surgery patients,including four aspects: adapting to role transition changes,mastering disease care knowledge in stages,caregivers attaching importance to self-management and caregivers taking the initiative to communicate with patients.The recovery rates of the two rounds of expert letter consultation were 93.8% and 100%.The expert authority coefficient was 0.917,The Kendall coefficient of the first round of expert correspondence consultation was 0.091~0.179,and the Kendall coefficient of the second round was higher than the first round,0.134~0.200(P<0.05).Conclusions1.This study investigated the caregiving readiness of family caregivers for breast cancer surgery patients,and the results showed that the caregiving readiness of family caregivers for breast cancer surgery patients was at a medium level,not fully prepared to take care of patients,and was affected by demographic factors,family members’ disease uncertainty and caregivers’ psychological factors.Clinical medical staff should pay attention to the readiness of caregivers and provide timely support to ensure the readiness of caregivers,so as to improve the recovery process and quality of life of patients undergoing breast cancer surgery.2.Based on the current investigation,literature review and expert letter consultation,the education program of family caregiver role preparation for breast cancer surgery patients,which covers adapting to the role transition,mastering nursing knowledge,proactively understanding patients’ needs and attaching importance to their own health,is preliminarily constructed,so as to conduct intervention research on family caregiver readiness for breast cancer surgery patients in the future.Improving the caregiving readiness of family caregivers provides references and references.
