ObjectiveBy using surface electromyography detection technology,the immediate effect of acupuncture on patients with upper limb spasm after stroke was observed,which provided a basis for clinical application of acupuncture on spastic paralysis after stroke.MethodA simple randomized,single-center,single-blind observation clinical trial protocol was used.Sixty subjects who met the inclusion criteria for upper limb spasm during stroke recovery period were randomly assigned to the combination of exterior and interior meridians group(30 cases)and the combination of yang meridians group(30 cases)according to the order of treatment and inclusion.The acupoints of hand yangming meridian and hand taiyin meridian(jianyu,quchi,hegu,tianfu,chize,taiyuan)were selected for acupuncture in the combination of exterior and interior meridians group,and the acupoints of hand yangming meridian(jianyu,quchi,hegu)were selected for acupuncture in the combination of yang meridians group,30 minutes/time,once for treatment.Before and after treatment,surface electromyography(sEMG)and modified ashworth scale(MAS)were evaluated,and adverse reactions during treatment were observed and recorded.After the treatment,the data were statistically processed and analyzed to draw conclusions.Results(1)Comparison of surface electromyography signals before and after treatment in the combination of exterior and interior meridians group.After treatment,the integral electromyography(iEMG)and root mean square(RMS)of biceps brachii decreased(P<0.01),and the iEMG and RMS of triceps brachii increased(P<0.05).(2)The iEMG and RMS of biceps brachii were decreased after treatment in the yang meridian acupoint group(P<0.05).There was no significant change in iEMG and RMS of triceps brachii(P>0.05),and the difference was statistically significant before and after treatment.(3)After treatment,the decrease of iEMG and RMS of biceps brachii in the exterior-interior meridian combination group was lower than that in the yang meridian combination group,and the increase of iEMG and RMS of triceps brachii medial/lateral head was higher than that in the yang meridian combination group,the differences were statistically significant(P<0.05).(4)There was no significant change in the modified Ashworth score before and after treatment in the combination of exterior and interior meridians group and the combination of yang meridians group(P>0.05).Conclusions(1)The iEMG and RMS of biceps brachii can be reduced in both the exterior-interior meridian combination group and the yang meridian combination group,and the flexor spasm can be alleviated,but the exterior-interior meridian combination method is superior to the simple acupuncture of the yang meridian.(2)The combination of exterior and interior meridians can improve the iEMG and RMS of the triceps brachii,and can improve the muscle strength of the extensor.The effect on iEMG and RMS is better than that of the yang meridian combination group.(3)Both groups can change the EMG value of the surface electromyography,while the modified Ashworth scale does not change significantly,suggesting that the response of the surface electromyography to limb spasm is better than that of the modified Ashworth scale,which provides a basis for the treatment of upper limb spasm after stroke. |
