Objective: By collecting clinical cases of patients with primary nephrotic syndrome treated by chief physician Yu Xiumei,and using the auxiliary platform of traditional Chinese medicine inheritance to carry out data mining,sorting and analysis,to explore the medication rules of chief physician Yu Xiumei in treating the disease,so as to better inherit and promote the academic thoughts and clinical experience of famous physician and guide clinical practice.Method: Collect the case data of patients with primary nephrotic syndrome treated by Chief Physician Yu Xiumei in the outpatient department of Liaocheng Hospital of Traditional Chinese Medicine from October 2020 t-o November 2022,and according to the inclusion and exclusion criteria,enter the eligible case data into the "Traditional Chinese Medicine Inheritance Assistance Platform(V2.5)",and establish the "Original Data Database of Chief Physician Yu Xiumei’s Treatment of Primary Nephrotic Syndrome",Use the platform’s "statistical report" and "data analysis" system to collate and mine data,obtain drug frequency,drug core combination and new prescription,and export the results for discussion.Results:1.A total of 118 patients were included in this study,including 80 males and 38 females;The 49-58 age group has the largest number of people.2.Of the 118 patients included in this study,Membrane nephropathy is the most common pathological type,with 85 cases;There are 31 main syndrome types,and the first three are:dampheat accumulation,stagnation of dampness and turbidity,liver depression and qi stagnation;There are 17 kinds of secondary syndrome elements,and the first three are blood stasis,excessive fluid and yang deficiency.3.A total of 624 prescriptions were included in this study,involving231 traditional Chinese medicines,with a cumulative total frequency of 75-79 times.There are 24 traditional Chinese medicines with a frequency of>100.According to the frequency,they are ranked in descending order.The top ten are:rosa laevigata,poria cocos,licorice,astragalus,Bixie,tangerine peel,plantain seed,corn beard,salvia miltiorrhiza,and astragalus;Traditional Chinese medicine with a frequency of>100 involves 9 kinds of drugs,and the first three drugs with a frequency are:drugs for tonifying deficiency,drugs for promoting water and permeability,and drugs for regulating qi;There are 5 kinds of drug properties,mainly warm,mild and cold;There are 6 kinds of medicinal taste,mainly sweet,bitter and acrid;There are 12 meridians,mainly spleen,kidney,liver,lung and stomach.4.This study summarized 48 commonly used drug combinations,mainly including astragalus complanatus-rosa laevigata,poria cocos-rosa laevigata,glycyrrhiza uralensis-rosa laevigata,Bixie-rosa laevigata,maize whisker-rosa laevigata,etc.5.This study has excavated 17 new prescriptions,among which the ones with higher clinical value are:Tinglizi-wax gourd skin-Shuihonghuazi-poly-porus umbellatus-Dafupi,licorice-bupleurum chinense-Wuyao-paeonia lactif-loramint,phellodendron amurense-anemarrhena asphodeloides-coix seed-atractylodes lanceolata-Chuanniuxi,alisma orientalis-yamporia cocos-acorus calamus-Bixie-Yizhiren.Conclusion: The results of this study indicate that the basic pathogenesis of nephrotic syndrome is deficiency in origin and excess in superficislity.The deficiency in origin is mainly the deficiency of qi and yang of the spleen and kidney,and excess in superficislity include blood stasis,excessive fluid,and dampness turbidity,this is consistent with the academic thinking of the mentor.Tutors often treat this disease from a holistic perspective in clinical practice,the elimination of pathogenic factors and the strengthening the body resistance,the balance of yin and yang,and the treatment of both symptoms and symptoms.Emphasis is placed on the origin of the congenital foundation:kidney and source of acquired constitution:spleen in succession,emphasizing the view of the homology of liver and kidney,and good at differentiation and treatment of nephrotic syndrome from the liver.
