Objective:To explore the medication and prescription characteristics of Professor Zhou Caiyun in the treatment of SLE at remission stage,and search for the potential mechanism of the core prescription.Methods:1.Research on data miningThe data mining method was used to count the ward data of patients with systemic lupus erythematosus at remission stage which were treated by Professor Zhou Caiyun from January 2015 to December 2021.The data were analyzed by statistics,association,clustering,etc.,and the academic thoughts and experience of Professor Zhou Caiyun in treating SLE at remission stage were summarized to obtain the core prescription.2.Prediction of network pharmacological mechanismCombining the network pharmacology method to predict the effective and active ingredients,possible targets and signal pathways of the core prescription in treating SLE at remission stage,and use molecular docking technology to verify the possibility and effectiveness of the core prescription in treating SLE at remission stage.Results:1.Research results of data miningIn this study,90 patients with SLE at remission stage were included,the ratio of male to female was 1:14,and the age distribution of patients was concentrated in the range of 40-49 years old,accounting for 26.67%;The largest proportion of patients with disease was course>5 years,50%;The main comorbidities of the patients were osteoporosis(48.89%),lupus nephritis(32.22%),hypertension(23.33%),etc;The dose of glucocorticoid in most patients was≤10mg/d;54 clinical symptoms occured in the the study,including arthralgia(46.67%),fatigue(38.89%),poor sleep(33.33%),butterfly facial erythema(28.89%),mouth dryness(25.56%),anorexia(15.56%),cough(13.33%),sweating(12.22%),hair loss(12.22%),etc;10 kinds of tongue images,8 kinds of tongue coating images and 16 kinds of pulse images were included,referring dark red tongue(45.56%),light red tongue(18.89%),red tongue(17.78%),yellow and greasy tongue(31.11%),thin and white tongue coating(26.67%),thin and yellow tongue coating(18.89%),wiry thready pulse(15.56%),thready slippery pulse(14.44%),etc;12 types of syndrome distribution occured in the study,including yin deficiency and internal heat,stasis and heat blockage,hepatic-kidney yin deficiency.and excessive toxic heat.Therefore,the treatment were mainly to nourish yin and clear heat,tonify liver and kidney,promote blood circulation and remove stasis,clear heat and remove dampness,and detoxify;The statistical analysis of traditional Chinese medicine involved 135 kinds.High-frequency drugs included poria cocos.raw earth,peony skin.etc.The four drugs were mainly cold,accounting for 43.35%,the five flavors were mainly sweet.accounting for 44.46%,and the meridians were mainly kidney meridians;Correlation analysis showed that the core drug pairs included yam-poria cocos,poria cocos-thicken fruit,anemarrhena asphodeloidesphellodendron amurense,etc;Cluster analysis showed that 6 groups of new prescriptions were used to treat patients with SLE in different syndrome types and symptoms;The complex network analysis showed that the core prescriptions were:poria cocos,raw rehmannia,peony bark,cornus officinalis,Chinese yam,phellodendron amurense,anemarrhena asphodeloides,lily,and gorgon.2.Results of Network pharmacological mechanism predictionThe core prescription of nine drugs involved 136 kinds of active ingredients and 419 corresponding targets;A total of 505 systemic lupus erythematosus disease targets were collected.After mapping the active ingredient target and the disease target,46 common targets were obtained,involving 49 kinds of main active ingredients,mainly including quercetin,β-Sitosterol,kaempferol,stigmasterol,diosgenin,etc;The protein-protein interaction network diagram showed that the core protein includes IL-1B,IL6,CXCL8,TNF,VEGFA,MMP9,etc;GO enrichment analysis showed that the core gene had 2136 entries,involving 1982 BP,211 CC and 102 MF;The results of KEGG enrichment analysis showed that the core prescription acted on SLE at remission stage mainly through PI3K-Akt signal pathway,IL-17 signal pathway,TNF signal pathway,etc;Molecular docking results verified the possibility and effectiveness of core prescription in treating SLE at remission stage.Conclusion:1.Professor Zhou Caiyun considered that the pathogenesis of SLE at remission stage was characterized by the deficiency of the original and the combination of the actual and the deficiency;According to the data mining results,the disease location was liver,kidney and related to the the spleen;The key pathogenesis was the deficiency of the liver and kidney;The pathological factors were "dampness,heat,poison and blood stasis";The main treatments of the prescription were to nourish yin and clear heat,replenish the liver and kidney,and take into account the diuresis and detoxification,and promote blood circulation and remove blood stasis.2.The core prescription treated the remission stage of systemic lupus erythematosus by giving full play to its anti-inflammatory,immune regulation,antioxidant stress,and vascular endothelial function through the advantages of multi-component,multi-channel,and multi-target,providing a theoretical basis for its mechanism of action. |
