| Purpose:In this study,the quality of life of the elderly in MCR elderly care institutions was investigated by cross-sectional survey;This paper analyzes the influencing factors of the quality of life of the elderly in MCR from five aspects of the health ecology model;The influencing factors of the elderly in the construction of the elderly care institutions MCR This study is to understand the influencing factors,and provide help to improve the health level of the elderly in the elderly care institution MCR,improve their quality of life,and realize the"healthy aging"in the"2030 Healthy life,and realizing the"healthy aging"in the"2030 Healthy China"plan.Methods:From September 2021 to June 2022,13 elderly care institutions in Shenyang were selected using convenient sampling method,and 251 MCR elderly people who met the inclusion criteria were investigated with a questionnaire.The research tool used general data questionnaire;Subjective cognitive decline questionnaire(SCD-Q9)was used to investigate the cognitive function of the subjects;The simple international fall efficiency energy scale(FES-1)was used to assess the subjects’fall fear;Perceived social support scale(PSSS)was used to assess the social support felt by the subjects;The depression status of the subjects was assessed with the Simplified Geriatric Depression Scale(GDS-15);The ability of daily living(ADL)was used to evaluate the study subjects’ability of daily living(ADL);The quality of life of the elderly in elderly care institutions was assessed using the quality of life scale;The maximum walking speed of 10 meters(10 MWS)was used to measure the walking speed of the research object.SPSS26.0 was used to establish the database and carry out descriptive statistical analysis,single factor analysis and hierarchical regression analysis.AMOS 26.0software was used to build a structural equation model of the influencing factors on the quality of life of the elderly in the elderly care institutions MCR.Results:1.The total score of life quality of the elderly in the MCR of elderly care institutions is(147.50±13.84),including physical health(11.87±1.96),food satisfaction(19.61±3.12),sense of security(16.76±2.20),environmental comfort(18.07±2.17),decision-making autonomy(11.54±1.10),activity participation(12.38±2.66),interpersonal relationship(21.80±2.44),family intimacy(14.91±2.12),and mental health(20.55±2.50).2.Single factor analysis results:the quality of life of the elderly in the elderly care institution MCR in terms of gender,medication,type of chronic disease,whether there has been a fall in the past year,whether it is personal will to stay in the elderly care institution,reasons for staying in the elderly care institution,frequency of exercise,sleep time,depression,fall efficacy,number of family visits,social support,education level,monthly income,star level of the elderly care institution The difference in economic sources was statistically significant(P<0.05).3.Stratum regression shows the type of chronic disease,depression,fall efficacy,social support,education level,and star level of elderly care institutions in the past year.The factors at five levels of the health ecology model jointly explain 90.9%of the total variation of the quality of life of the elderly in the MCR elderly care institutions.4.The results of the correlation analysis between social support,depression,fall efficacy and quality of life of the elderly in the elderly care institution MCR showed that:social support was significantly positively correlated with fall efficacy and quality of life(P<0.05),and was significantly negatively correlated with depression(P<0.05);Fall efficacy was negatively correlated with depression(P<0.05)and positively correlated with quality of life(P<0.05);There was a significant negative correlation between depression and quality of life(P<0.05).The structural equation model was constructed with social support as the external dependent variable,fall efficacy and depression as the intermediary variables,and the quality of life of the elderly in MCR as the internal dependent variable.The results show that the model fits well,and the fitness indicators of the model are:χ~2/df=2.649,CFI=0.938,IFI=0.938,TLI=0.921,RMSEA=0.078,GFI=0.901,AGFI=0.902.The intermediary relationship analysis results of the model show that the direct effect of social support on quality of life is0.222,95%confidence interval(0.131,0.316),and the direct effect accounts for 24.89%of the total effect;The chain mediated effect value of depression and fall efficacy was0.018,95%confidence interval(0.034,0.085),indicating that the chain mediated path was significant,and the chain mediated effect accounted for 2.02%of the total effect.The direct effect of social support on depression was-0.166,95%confidence interval(-0.291,-0.043),and the direct effect accounted for 34.66%of the total effect.Conclusion:1.The quality of life of the elderly in the elderly care institution MCR is at a medium level.2.Gender,drug use,type of chronic disease,whether there has been a fall in the past year,whether it is personal will to stay in an elderly care institution,reasons for staying in an elderly care institution,frequency of exercise,sleep time,depression,fall efficacy,number of family visits,social support,education level,monthly income,star level of the elderly care institution,and economic source will affect the quality of life of the elderly in the elderly care institution MCR.Further hierarchical regression analysis showed that the type of chronic diseases,depression,fall efficacy,social support,education level,and the star level of elderly care institutions were the main factors affecting the quality of life of the elderly in MCR.3.The results of structural equation model show that social support not only directly and positively affects the quality of life of the elderly in the elderly care institution MCR,but also can affect the quality of life through the chain mediation of fall efficacy,fall efficacy and depression.Social support not only directly affects depression,but also can affect depression through the mediation of fall efficacy. |
