Objective: To observe the efficacy and safety of Xiang Sha Yang Gastric San plus and minus Huo Ma Ren in the treatment of cancer pain patients with opioid-induced intestinal dysfunction,to assess the quality of life of patients before and after treatment,and to further analyze the correlation between gastrointestinal dysfunction and clinical traits.Methods: Patients with cancer pain who were prescribed opioids in the outpatient clinics of the two hospitals of the Affiliated Hospital of Southwest Medical University in 2021-2022 were selected and the population meeting the requirements was randomly divided into two groups by random assignment and controlled trial for comparative study.Based on the existing opioid analgesic regimen,the test group was treated with a combination of Xiang Sha Yang Gastric San plus and minus Fire Ma Ren(for patients with concomitant constipation),while the control group was treated with opioids for a total of 4weeks.Firstly,the general data of patients were recorded,including gender,age,stage,cancer type,KPS score,etc.Secondly,the main observation indexes included the improvement of nausea,vomiting,constipation,bloating and diarrhea,and the symptoms were digitized by CTCAE adverse reaction criteria and Rome III constipation diagnostic criteria,and the symptom scores of the two groups were observed before and after the test;The secondary observation indicators were the change of pain intensity,change in the number of pain outbreaks,quality of life,and KPS score between the two groups before and after the trial.Through the detection of safety indicators such as blood routine and liver and kidney function,we can observe the changes before and after the test.Finally,the correlation between high and low scores and clinical traits was explored through the score of gastrointestinal symptoms.Results: After trial,the GI symptoms in the test group improved significantly,with the most significant changes in constipation,nausea and bloating diarrhea disorders,and the statistical significance of these changes were greater than in the control group(P<0.05),while the changes in vomiting disorders were not statistically significant(P>0.05).Before and after treatment,there was no difference in pain intensity change(NRS)between the two groups,and both remained below 4 points.Compared with the control group,the survival level of the patients in the experimental group was significantly improved,and the KPS score was also statistically significant,(P<0.05).There were no differences in blood routine and liver and kidney function in the control group and experimental group before and after treatment.The gastrointestinal symptom score(referred to as the symptom score)was related to the type of cancer and the analgesic drugs used(P<0.05),which was specifically manifested as a high score of gastrointestinal tumor symptoms,while the low symptom score of patients using fentanyl transdermal patch was not related to gender,age,stage,treatment mode,and whether there was bone metastasis.Conculation: Gastrointestinal dysfunction caused by opioids is treated by using Xiang Sha Yang Gastrodisian plus and minus fire mahogany.By using this method,gastrointestinal reactions,including nausea,bloating,diarrhea and constipation,can be effectively relieved,thus alleviating the patient’s unpleasant feelings.It can also improve the quality of life.In addition,Xiang Sha Yang Gastrodia has a safety profile and has no effect on patients’ bone marrow,liver and kidney functions.It can be used in the treatment of opioid side effects. |
