Construction Of Continuous Nursing Plan For Patients With Acute Myocardial Infarction After PCI Based On IMB Model

ObjectiveTo explore the continuous nursing scheme of patients with acute myocardial infarction after PCI.On the basis of literature research and qualitative interview methods,Delphi method was adopted to construct the continuous nursing plan of patients with acute myocardial infarction after PCI based on IMB model,which provided the basis and guidance for clinical practice.MethodsIn this study,the qualitative interview method was used to understand the real nursing needs of AMI patients after PCI discharge and the views and suggestions of medical staff on continuity care.Through literature retrieval and analysis of Chinese and English databases at home and abroad,the current situation of continuity care after PCI was grasped.The IMB model was used as the theoretical guidance model.Build a preliminary framework for continuity of care.Then Delphi expert letter consultation method was used to establish the final plan of continuity care for patients with acute myocardial infarction after PCI based on IMB model through two rounds of expert letter consultation.Statistical tools in this study include Excel 2019,SPSS 23.0 and other statistical methods using statistical description,statistical inference,and the test standard α<0.05.Results1.In this study,12 AMI patients undergoing PCI were interviewed,and five themes were extracted:lack of disease-related knowledge,high requirements for continuity of care,multi-level care needs,diversified follow-up methods,and large differences in frequency of follow-up.Through the interview of 10 medical staff in the department of cardiology,summed up three themes:the necessity of carrying out continuity of care,the implementation of continuity of care problems and improvement measures,the improvement of continuity of care.Based on clinical literature analysis,a total of 43 literatures were included for research and analysis to determine the first draft of the continuity of care program.2.Based on the IMB model of AMI patients’ continuous nursing needs after PCI,this study takes the IMB model as the theoretical guidance,and clarifies the nursing content from four dimensions of information guidance,motivation guidance,behavioral skill guidance and continuous nursing implementation.According to the entry screening criteria,combined with the expert letter consultation,Finally,an IMB model-based continuity care program for AMI patients after PCI was established,including 4 first-level indicators,15 second-level indicators and 65 third-level indicators.All nursing contents fit the physical and psychological aspects of patients after PCI for acute myocardial infarction to achieve comprehensive continuity care.The result of this letter indicates that the enthusiasm,authority,concentration and coordination of experts are good,and the research results are scientific.Conclusions1.Through semi-structured interview,it was found that most patients have a high demand for continuity of care,most patients do not know AMI and PCI related content,and the content of home self-management is not clear.Medical staff should systematically understand and master the actual needs of clinical patients after discharge home care,attach importance to nursing experience,and give patients personalized guidance.2.The continuous care plan for patients with acute myocardial infarction after PCI constructed by this study has high representativeness and authority.The research methods are scientific and rigorous,and the consulting results are scientific and reliable.The nursing plan integrates the health education content of clinical information education,motivation stimulation and behavioral skill guidance into the implementation of continuous care.Therefore,it can improve the self-management ability of patients and improve the prognosis of patients.It has certain clinical adaptability and can provide reference for clinical work.
