| ObjectiveThis study was to investigate and analyze the prevalence of low back pain and associated factors in residents over 18 years of age in Xiushan County,Chongqing,to assess the degree of functional impairment in low back pain patients with different characteristics,and to provide epidemiological information of low back pain,and provide references for the prevention and rehabilitation of low back pain.MethodsThis study conducted a cross-sectional survey about residents’ low back pain in Xiushan County,Chongqing,China,from May 2021 to June 2021,and the study population was residents aged 18 years or older in Xiushan County.The survey included demographic characteristics,low back pain prevalence,Roland-Morris Dysfunction Questionnaire(RMDQ),Oswestry Dysfunction Index(ODI),Japanese Orthopaedic Association score(JOA),and Numerical Rating Scale for Pain(NRS).Results(1)A total of 1820 residents were recruited in this study,and after excluding ineligible questionnaires,1704 resident questionnaires were included in the statistical analysis.30.5%of residents had low back pain,and the prevalence of low back pain was 26.3%in flatland areas and 35.6%in mountainous areas;living in mountainous areas(OR 1.402,95%CI 1.085-1.812),advanced age(45-59 years OR 1.761,95%CI 1.264-2.455;60-74 years OR 2.309,95%CI 1.566-3.406;≥75 years OR 2.054,95%CI 1.178-3.579),married or remarried(OR 1.872,95%CI 1.089-3.217),divorced or widowed(OR 2.705,95%CI 1.364-5.366),moderate labor intensity(OR 1.365,95%CI 1.052-1.771),frequent bending(OR 1.617,95%CI 1.108-2.360),and depressed mood(OR 1.610,95%CI 1.246-2.080)were potential risk factors for low back pain among residents of Xiushan County,Chongqing.Regular exercise(OR 0.662,95%CI 0.497-0.881)and high school education level(OR 0.563,95%CI 0.319-0.996)were potential protective factors for low back pain in Xiushan County residents.For residents of flat areas,45-74 years of age,college or higher education level,and occasionally or frequently feeling sad or grieving were potential risk factors for low back pain,while regular exercise was a potential protective factor for low back pain;for residents of mountainous areas,60-74 years of age,75 years of age and older,married or remarried,divorced or widowed,fond of playing cards,frequently bending over and occasionally feeling sad or grieving were potential risk factors for low back pain,and Tujia and housewives were potential protective factors for low back pain.In addition,patients with low back pain in mountainous areas had more severe functional impairment and pain levels compared to patients with low back pain in flat areas(P=0.012;P<0.001).(2)A separate analysis of 542 older adults(aged 60 years and older)revealed that the overall prevalence of low back pain in older adults was 37.5%.The elderly were divided into three groups of 60-69,70-79,and ≥80 years old,and the degree of pain and dysfunction was compared among the elderly with low back pain of different ages and genders,and it was found that there was no significant difference in the degree of pain and dysfunction among the elderly with low back pain of different ages and genders.Multivariate analysis showed that being economically underdeveloped(OR 1.732,95%CI 1.123-2.671),divorced or widowed(OR 6.035,95%CI 1.697-21.465),laboring or working 4-8 hours per day(OR 2.161,95%CI 1.232-3.789),occasionally feeling sad or grieving(OR 1.913,95%CI 1.200-3.048)were potential risk factors for low back pain and regular exercise(OR 0.573,95%CI 0.354-0.928)were potential protective factors for low back pain.Conclusions(1)The prevalence of low back pain in mountainous areas and the degree of dysfunction were higher among residents of Xiushan County,Chongqing than in flatland areas,which may be related to settling in mountainous areas,advanced age,lifestyle and occupational characteristics.Appropriate preventive measures should be taken to reduce the economic and health burden caused by low back pain.(2)The prevalence of low back pain among the elderly in Xiushan County is higher than the overall global average prevalence of low back pain,and factors such as economic underdevelopment and depressed mood are closely related to the prevalence of low back pain,and proper exercise may prevent low back pain. |
