| Objective To explore the academic ideas and principles of He’s Gynecological herbal paste and to explore the Medication Law in order to provide reference for clinical prescriptions and to promote the academic inheritance of He’s Gynecological School.Methods Combined with the method of theoretical research and data mining.(1)Theoretical research:through literature research and clinical shadowing,combed the lineage of He’s gynecological inheritance and summarized the academic ideas of He’s gynecological herbal paste.(2)Data mining:Collect the medical records of He’s Gynecological herbal paste from October 2018 to January 2022 in Hangzhou Hospital of Traditional Chinese Medicine through HIS system.Screened strictly according to the inclusion and exclusion criteria,and then establish a database.Frequency analysis,association rule analysis and cluster analysis were used to uncover Medication Law of He’s Gynecological herbal paste.Results(1)He’s gynaecology is good at using herbal paste to recuperate gynecological diseases,and the academic ideas include:①syndrome differentiation,three-categories of etiologic factors;②Yin and Yang in equilibrium,achieve new energy;③Sufficient Healthy-Qi inside the body;④ strengthening vital qi to eliminate pathogenic factor,reinforcement and elimination in combination;⑤tonic liver and kidney;⑥reinforcing spleen and stomach;⑦recharge eight extra-channel;the extra channels,coordinating Chong and Conception Vessels;⑧mild prescription and flexible medication.(2)In this study,188 prescriptions of He’s gynecological herbal paste were included,involving 288 varieties and 6489 times of total frequency,among which 274 varieties of Traditional Chinese Medicines prepared in ready-to-use forms were used 5305 times(including 11 varieties of costly medicinal medicine were used 255 times),5 varieties of gelatins were used 462 times,and 9 varieties of sugar and excipients were used 722 times.The frequency of gelatin use is Colla Corii Asini,Colla Carapacis et Plastri Testudinis glue,Cervi Cornus Colla,Colla carapacis trionycis and Oxhide gelatin in turn.The use of gelatins in the herbal paste ranges from single glue to the combination of four gelatins types,and the largest proportion is the combination of"Colla Corii Asini+Colla Carapacis et Plastri Testudinis glue+Cervi Cornus Colla".The four properties of the Traditional Chinese Medicines prepared in ready-to-use forms are mainly distributed in flat,mild and slightly warm,the five flavors are mainly distributed in sweet,pungent and bitter,and then the normalization channels are mainly distributed in liver,kidney and spleen.The efficacy of Traditional Chinese Medicines prepared in ready-to-use forms were mainly tonic,followed by blood-activating and stasis-resolving medicinal,relieve uneasiness of mind and body tranquilization,regulate the flow of vital energy and remove obstruction toit and antipyretic.The analysis of the association rules of high-frequency drugs revealed that He’s gynecological herbal paste used Colla Corii Asini-Cuscutae Semen-Lycii Fructus-Angelicae Sinensis Radix-Paeoniae Radix Alba as the 5-drug association ring.The most extensive and stable drug pair is "Colla Corii Asini-Angelicae Sinensis Radix" in the second-order association,"Colla Corii Asini-Angelicae Sinensis Radix-Lycii Fructus" in the third-order association,and "Colla Corii Asini-Angelicae Sinensis Radix-Lycii FructusCuscutae Semen" in the fourth-order association.Cluster analysis was performed on the top 34 drugs in terms of frequency of use,and five clusters were obtained.Conclusion(1)"qi and blood deficiency,liver-kidney deficiency and cold coagulation and blood stasis" are the main pathogenesis of gynecological diseases.He’s gynecology proposed that the main principle of herbal paste prescription was to tonify the deficiency and cure the disease.Take "Tonifie qi and blood replenishes,Tonic liver and kidneys,warm yang and remove blood stasis" as the treatment method to reconcile qi and blood,supplement yin and yang,and treat deficiency and excess simultaneously.(2)The prescription of He’s Gynecological herbal paste strictly adheres to the pathogenesis,mainly uses the medicine of tonifying deficiency,promoting blood circulation and removing blood stasis.When prescribing,the method of "Invigoration,Warm,Eliminate,and Clear heat" of the eight laws were often used,accompanied by tranquillization,so as to achieve the effects of benefiting qi for promoting production of blood,coordinating yin and yang,nourishing liver and kidney,reinforcing spleen and stomach,and promoting blood flow and resolves blood stasis.In addition,Colla Corii Asini is mainly used in the prescription,and then Colla Carapacis et Plastri Testudinis glue,Cervi Cornus Colla,Colla carapacis trionycis were flexibly selected according to the patients’ yin and yang conditions,so as to achieve the effect of replenishing qi,blood,yin and yang.The prescription of herbal paste conformed to the academic ideas of He’s Gynecology that "tonify deficiency,coordinate yin and yang,nourish liver and kidney,reinforce spleen and stomach,and then reinforcement and elimination in combination",and also corresponded to the main pathogenesis of gynecological diseases.All in all,the herbal paste has distinctive specialist characteristics. |
