Objective: To explore the feasibility and effectiveness of methylene blue for detecting anastomotic leakage by applying the methylene blue test in distal radical gastrectomy for gastric cancer.Methods: This study randomly selected 76 patients with laparoscopic distal D2 radical gastrectomy for gastric cancer from April 1,2020 to February 1,2022 in the Department of Gastrointestinal Surgery,Qinghai University Affiliated Hospital.All patients were performed by the same group of doctors.The digestive tract reconstruction was performed by Billroth I gastroduodenal anastomosis.After the anastomosis,the experimental group used intestinal forceps to close the intestinal tube about 1cm below the anastomosis,and put the gastric tube into the stomach.,and then use 1 piece of methylene blue(20 mg)and 400-500 ml of normal saline to inject into the remnant stomach through a syringe until it is filled,and observe whether there is methylene blue exudation around the anastomosis through laparoscopy.repair.To increase comparability,the control group was injected with400-500 ml of saline during surgery.The operation time data of the two groups of patients were collected after the operation,the time of the first anal exhaust of the patients after the operation and the hospitalization expenses incurred when they were discharged from the hospital were observed.Postgastroparesis syndrome(PGS)and other clinical manifestations.Results: 1.The baseline data of the experimental group and the control group were P>0.05,and the difference was not statistically significant.2.Compared with the experimental group and the control group,the incidence of postoperative anastomotic leakage was lower than that of the control group,P<0.05,and there was a difference.Statistical significance;at the same time,the postoperative liver function and renal function damage between the two groups were P>0.05,and the difference was not statistically significant;3.Compared with the control group and the experimental group,the operation time was P>0.05,and the difference was not statistically significant.;postoperative anal first exhaust time P < 0.001,there is a statistical difference;postoperative hospitalization time P < 0.001,there is a statistical difference;4.Compared with the experimental group and the control group,the postoperative intra-abdominal infection,PGS,the incidence of wound infection P>0.05,no statistical difference.Conclusion: 1.Meilan test has certain value in reducing the occurrence of postoperative anastomotic leakage in laparoscopic radical gastrectomy for distal gastric cancer;At the same time,the postoperative ventilation time was shortened,the length of hospitalization was reduced,and the hospitalization cost was saved.2.The use of low-dose Meilan has no obvious damage to the liver and kidney function of patients,and has no obvious relationship with postoperative complications such as incision infection,intraperitoneal infection and gastroparalysis;Meilan reagent is safe and effective and can be used in low dose in clinic.
