Objective: To explore the clinical significance of serum IL-1β,IL-6,and IL-17 in patients with brucellosis osteoarthritis and to study the relationship between the degree of brucellosis osteoarthritis and IL-1β,IL-6,and IL-17,And to study the mechanism of brucellosis osteoarthritis.Methods: This experiment is a case-control study.From January 2021 to January 2022,the Affiliated Hospital of Inner Mongolia Medical University strictly selected 20 patients with brucellosis with a course of more than six months as the chronic brucellosis group and 20 patients with brucellosis complicated with osteoarthritis with a course of more than six months as the brucellosis osteoarthritis group according to the inclusion and exclusion criteria of the subject,and then selected the local health checkups in the outpatient department in the same period,As the control group.The levels of IL-1β,IL-6,and IL-17 in peripheral blood of the three groups were detected by enzyme-linked immunosorbent assay.The levels of C-reactive protein(CRP),procalcitonin(PCT),and erythrocyte sedimentation rate(ESR)were measured by DXC800 automatic biochemical analyzer,colloidal gold colorimetry,and erythrocyte sedimentation rate(ESR).The multivariate logistic regression model was used to analyze the relevant independent risk factors of brucellosis osteoarthritis,calculate the area under the curve(AUC),and draw the subject working characteristic curve(ROC curve)to analyze the diagnostic efficacy of blood indexes in patients with chronic brucellosis and brucellosis osteoarthritis.Finally,the results were compared and analyzed.Results: the average levels of IL-1β,IL-6,IL-17,CRP,PCT and ESR in the control group,chronic brucellosis group and brucellosis osteoarthritis group were 11.609 ± 1.764(111.900 ±18.145;133.000 ± 36.748)pg / ml,14.153±1.294(43.050 ± 13.570;54.500 ± 21.617)pg / ml,6.955 ± 1.429(117.950 ± 45.223;146.050 ± 49.163)pg / ml,5.779 ± 1.929(19.493 ± 7.325;22.187 ± 4.802)pg / ml,respectively,0.045 ± 0.017(3.162 ± 0.370;3.171 ± 0.378)mg / L and 12.050 ± 2.685(37.400 ± 8.917;41.050 ± 7.810)mm / h.There was significant difference between the six indexes(P < 0.05).Compared with the control group,PCT,CRP and ESR in chronic brucellosis group and brucellosis osteoarthritis group were significantly higher;IL-6,IL-17 and IL-1β in brucellosis osteoarthritis group were significantly higher than those in chronic brucellosis group and control group;Compared with the control group,IL-6,IL-17 and IL-1β in chronic brucellosis group increased significantly(P < 0.05).Multivariate logistic regression analysis showed that CRP(OR: 1.142,95% CI: 1.005 ~ 1.297,P < 0.05),ESR(OR:1.092,95% CI: 1.040 ~ 1.366,P < 0.05),IL-6(OR: 1.023,95% CI: 1.001 ~ 1.046,P < 0.05)and IL-17(OR: 1.028,95% CI: 0.995 ~ 1.061,P < 0.05)were independent risk factors for brucellosis osteoarthritis.The ROC curve analysis showed that the AUC of IL-1β,IL-6,IL-17 and IL-6 combined with IL-17 in the diagnosis of chronic brucellosis were 0.764(95% CI:0.626 ~ 0.901,P < 0.001),0.831(95% CI: 0.730 ~ 0.932,P < 0.001),0.768(95% CI: 0.646 ~0.890,P < 0.001)and 0.865(95% CI: 0.772 ~ 0.985,P < 0.001),respectively.The AUC could be increased to 0.893(95% CI: 0.810 ~ 0.976,P < 0.001).The AUCs of IL-1β,IL-6,IL-17 and IL-6 in the diagnosis of Brucella osteoarthropathy were 0.746(95% CI: 0.610 ~0.883,P < 0.001),0.781(95% CI: 0.655 ~ 0.907,P < 0.001),0.747(95% CI: 0.621 ~ 0.873,P < 0.001)and 0.830(95% CI: 0.727 ~ 0.933,P < 0.001).Combined detection of IL-1β,IL-6and IL-17 could increase the area under the curve to 0.880(95% CI: 0.794 ~ 0.966,P <0.001).Concussion:1.Serum PCT,CRP,ESR,IL-1β,IL-6,and IL-17 in patients with chronic brucellosis and brucellosis osteoarthritis were significantly higher than those in the control group,and the above indexes in patients with brucellosis osteoarthritis were significantly higher than those in patients with chronic brucellosis.2.Il-6,IL-17,CRP,and ESR are closely related to brucellosis osteoarthritis and are independent risk factors of brucellosis osteoarthritis.3.In the diagnosis of patients with chronic brucellosis and brucellosis osteoarthritis,the combined diagnostic efficacy of IL-1β,IL-6,and IL-17 are greater than that of the three individual indicators,which has a certain value for the early diagnosis of chronic brucellosis and brucellosis osteoarthritis. |
