| Research purposes:In this study,the change of CAP value was used as the evaluation standard for steatosis.By analyzing the relationship between the clinical characteristics and diagnosis and treatment methods of 114 patients with non-alcoholic fatty liver disease and the change of CAP value,the clinical characteristics of patients with significantly improved steatosis and related intervention methods were discussed.In order to provide a basis for the optimal management of NAFLD patients,and to provide guidance for the selection of clinical diagnosis and treatment plans and the evaluation of efficacy for patients with NAFLD.Rresearch method:In this study,the clinical data of 114 NAFLD patients who met the diagnostic criteria and inclusion and exclusion criteria were collected from December 2020 to February 2022 in the XXX.The 114 patients were divided into an effective group and an ineffective group according to whether the CAP value was reduced by 10%compared with that before treatment.The differences between the two groups included gender,age,height,weight,BMI,liver function(ALT,AST,GGT,TBIL),blood lipids(TG,HDL-C,LDL-C),fasting blood glucose,TCM syndrome types,and living habits improvement,,weight changes,serum index changes,use of drugs to improve metabolism,traditional Chinese medicine,etc.were analyzed to obtain the influencing factors of the curative effect,and then the statistically significant influencing factors were subjected to regression analysis to obtain the indicators of independent influencing factors(with P<0.05 was considered statistically significant)Result:1.A total of 114 patients were included in this study.After grouping according to the above criteria,the number of patients in the effective group was 49,the number of patients in the ineffective group was 65,and the proportion of the effective group was 43%;2.There was no statistical difference in gender,age,height,weight before treatment,and BMI between the two groups;there was no statistical distribution of ALT,AST,GGT,TBIL,TG,HDL-C,LDL-C,and fasting blood glucose levels between the two groups before treatment.academic differences;3.The CAP of the patients in the effective group before treatment was 345.12±30.80d B/m,and the CAP of the ineffective group before treatment was 324.45±37.76 d B/m.There was a significant difference in CAP between the two groups before treatment(t=3.217,P=0.002);4.The median weight change rate in the effective group was 3.99(2.25)%,and the median weight change rate in the ineffective group was 0.69(1.52)%.There was a significant difference in the distribution of the weight change rate between the two groups(Z=8.077,P<0.001),and there is a linear correlation between the weight change rate and the CAP值change rate,CAP change rate=0.221+3.495×weight change rate;5.The exercise improvement rate of the effective group was 85.7%,and the exercise improvement rate of the ineffective group was 15.4%,with a statistically significant difference(χ~2=55.705,P<0.001);the improvement rate of diet in the effective group was83.7%,and the improvement rate in the ineffective group was 35.4%,there is a significant statistical difference(χ~2=26.458,P<0.001);6.There were statistical differences in the distribution of△AST(Z=3.205,P=0.001),△TG(Z=2.244,P=0.025),and△fasting blood glucose(Z=2.019,P=0.044)between the two groups.There was no statistical difference in the distribution of△GGT,△TBIL,△HDL-C and△LDL-C;7.There was no statistical difference in the usage rates of metformin and lipid-lowering drugs between the two groups;8.There was no statistical difference in the distribution of TCM syndrome types between the two groups;9.There is no statistical difference in the use rate of traditional Chinese medicine between the two groups.Among the people who meet the improvement of exercise and diet,the use rate of traditional Chinese medicine in the effective group is 80.6%,and the use rate of traditional Chinese medicine in the ineffective group is 16.7%.There is a statistical difference between the two groups(χ~2=7.394,P=0.007);10.The CAP level,diet improvement,exercise improvement,△AST,△TG,and△fasting blood glucose were included in the logistic regression equation for multivariate analysis.The results showed that there were statistically significant differences in the efficacy of body weight change rate and exercise improvement on steatosis.The OR values were as follows:7.887,3.627.Conclusion:1.Although the level of CAP value before treatment is an influencing factor for the efficacy of NAFLD steatosis,further analysis after excluding the improvement factors of diet and exercise found that the level of CAP value before treatment may affect the efficacy of steatosis by affecting the improvement of diet and exercise;2.The rate of body weight change is an independent factor affecting the efficacy of steatosis,and there is a positive correlation between body weight change and CAP change;3.△AST,△TG and△fasting blood glucose are the influencing factors of the curative effect of steatosis,the improvement of AST,TG and fasting blood glucose can indicate the improvement of steatosis;4.The improvement of exercise and diet are related to the change of CAP,and the improvement of exercise is an independent influencing factor of the efficacy of steatosis;5.On the basis of diet improvement and exercise improvement,the use of traditional Chinese medicine can achieve greater CAP reduction and better improvement of adipose tissue. |
