At present,China is in the phase of rapid growth of population aging.According to the seventh census,China’s population aged 60 and over is 264.019 million,making of 18.7% of the population.Physical health is the premise of social communication and life of the elderly,but the overall health condition of the elderly in China is no room for optimism,and the proportion of illness is high,which leads to a large demand for medical services and a high utilization rate of the elderly.Therefore,this paper studies the utilization of medical services and the influencing factors of medical results of the elderly population.On the one hand,it deepens the understanding of the utilization of medical services for the elderly from all dimensions.On the other hand,identify the key factors that affect the medical outcomes of the elderly,and put forward feasible suggestions for the pain points in the utilization of medical services for the elderly.This paper is based on the tracking survey data of China’s health and elderly care in 2011,2013,2015 and 2018,this paper firstly conducts descriptive statistics on the individual characteristics,medical behaviors and medical outcomes of the survey respondents.Secondly,taking the health status and patient satisfaction of the respondents as the observation indicators of medical outcomes,spatial correlation analysis,clustering and outlier methods were used to analyze the aggregation and differences of medical outcomes in 28 provinces(cities)in four years.Thirdly,the panel ordinal logistic regression was used to determine the key factors affecting the health status of the surveyed subjects and patient satisfaction,which laid the foundation for the subsequent construction of the structural equation.Finally,based on Anderson’s theoretical model and using the cross-sectional data in 2018,founded on the contributory factor obtained by regression analysis,the observation variables of the environment are introduced to build a structural equation model to analyze the relationship between the four potential variables of environment,individual characteristics,medical behavior and medical outcomes.The following five main conclusions are drawn:First,the health problems of the elderly in China are frequent,and the health status is not optimistic.Since 2011,the proportion of self perceived health deterioration of the elderly reached more than 40%.47.7% of the elderly suffered from chronic diseases and 30.6% reported pain.The problem of overweight and obesity of the elderly is also becoming increasingly prominent,especially the younger elderly.The proportion of elderly smokers is still high.Second,the burden of medical expenses of the elderly in China is heavy,and there are gender and regional differences in the use of medical services.The utilization rate of medical services for the elderly is high.The outpatient use rate and use times of women are significantly higher than those of men,and the inpatient use rate in urban areas is 1.5 percentage points higher than that in rural areas.The expenses of outpatient service,hospitalization and drug purchase for the elderly are not low,especially the hospitalization expenses,up to 9745.3 yuan.Third,there are time and space differences in the medical results of the elderly in China.There are significant differences in time and space between provinces and cities in terms of health status and patient satisfaction.The overall level of health status is high in the eastern coastal areas and low in the central and western inland areas;The overall performance of patient satisfaction is high in the north and low in the south.In addition,with the increase of years,the health status of the elderly and patient satisfaction increased significantly.Fourth,medical results are affected by many factors,and there are great differences among groups with different characteristics.Medical results were affected by gender,age,residential address,education level,marital status,smoking and other factors.Fifth,the medical behavior and environment of the elderly significantly affect the health outcomes of the elderly.The higher the frequency of medical behavior,the worse the medical result;The worse the environment and resources,the greater the negative impact on the medical results of the elderly. |
