Objective Micro RNAs are small endogenous RNAs with the ability to inhibit posttranscriptional expression of protein-coding genes.Available evidence suggests that microRNAs play an important role in tumorigenesis and progression as the main regulator of multiple pathophysiological processes.The objective of this research was to provide new biological markers for the diagnosis,treatment efficacy and prognosis assessment of AML by measuring the expression levels of microRNA-106b-5p,Rab10 mRNA in the bone marrow and Rab10 protein in peripheral blood of healthy donors and patients with AML,to analyze the differences in the expression of microRNA-106b-5p,Rab10 mRNA and Rab10 proteins between healthy donors and patients with AML,the differences between the initial diagnosis,recurrence and complete remission phases of AML patients,as well as combined with the correlation analysis between the expression level of microRNA-106b-5p and the clinical characteristics and indicators of AML patients.Methods From June 2020 to December 2021,we collected a total of 100 bone marrow samples and peripheral blood samples(85 AML patients and 15 healthy bone marrow donors)from the Affiliated Hospital of Qingdao University.Among the 85 patients with AML,there were 31 initial diagnosis patients,14 patients with recurrence,and 40 patients with complete remission.We followed up with 25 initial diagnosis patients by checking the hospitalization history and follow-up check.The relative expression of microRNA-106b-5p and Rab10 mRNA in bone marrow of AML patients was tested by qRT-PCR,the relative expression of Rab10 protein in serum was detected by ELISA method,the expression difference between microRNA-106b-5p and Rab10 in different groups of patients was assessed,the chromosomal karyotype of initial diagnosed patients was analyzed by R tape technique.The expression levels of microRNA-106b-5p and Rab10 at different course stages of AML were analyzed by Kruskal-Wallis H.The diagnostic performance of microRNA-106b-5p and Rab10 in AML was evaluated by receiver operator characteristic curve(ROC).Survival analysis of microRNA-106b-5p and Rab10 expression levels was performed on patients with AML.The correlation between the expression of microRNA-106b-5p with clinical features and indicators in the initial diagnosis patients was analyzed.Results The qRT-PCR results showed that the expression of microRNA-106b-5p,Rab10 mRNA and Rab10 proteins was apparently different in each group(H=103.532,41.565,70.606,P<0.001),and which in the initial diagnosis group and recurrence groups were apparently higher than in complete remission group and healthy control group(U=1~123.000,30.000~106.000,0~21.000,P<0.001).The expression level of microRNA-106b-5p and Rab10 mRNA in initial group were apparently higher than in relapse group(U=93.000,564.000,P<0.001),while there was no marked difference in the expression level of Rab10 protein between the initial diagnosis and the relapsed patients(U=3153.000,P>0.05).There were no marked differences in the expression levels of microRNA-106b-5p,Rab10 mRNA and Rab10 proteins between subtypes(AML-M1-M1 to AML-M5)(H=5.268,1.538,8.221,P>0.05).Changes in chromosome karyotype in the initial patients were not correlated with the expression level of microRNA-106b-5p(P>0.05).Dynamic monitoring of 5 patients in the initial group found that the expression level of microRNA-106b-5p,Rab10 mRNA in the bone marrow and Rab10 protein in peripheral blood of each patient after complete remission of treatment were significantly reduced(Z=-3.408,-3.294,-3.206,P<0.05).The expression level of miR-106b-5p was closely related to the number of white blood cells at the time of initial diagnosis in patients with AML(P=0.029),but was not related to other clinical features(sex,age,number of red blood cells,number of platelets,hemoglobin content and proportion of blastocytes in the bone marrow)(P>0.05).Spearman correlation analysis showed a positive correlation between microRNA-106b-5p and Rab10 mRNA and Rab10 proteins(r=0.5,P<0.001;r=0.38,P<0.05).The ROC curve showed that both microRNA-106b-5p and Rab10 proteins had high diagnostic value for AML(AUC: 0.944,0.853;sensitivity: 94.4%,97.9%;specificity: 86.7%,60.0%).KaplanMeier survival curve showed that patients with high expression of microRNA-106b-5p and Rab10 proteins had smaller EFS(event free survival,refers to the time from the time of inclusion in the study group to the occurrence of any event,such as exacerbation after treatment,recurrence,and death)than that patients with low expression(n=25,P<0.05),indicating that patients with low expression of microRNA-106b-5p and Rab10 proteins were more stable and had better prognosis.Conclusion(1)The expression of miR-106b-5p and Rab10 proteins may have a certain correlation with the occurrence and development of AML.(2)The expression of miR-106b-5p and Rab10 proteins was significantly correlated with the number of white blood cells in patients with initial diagnosis of AML,but was not related to the karyotype,subtype of AML chromosomes.Patients with lower expression of miR-106b-5p and Rab10 proteins had longer EFS,more stable conditions and better prognosis.(3)Mi R-106b-5p expression is related to the expression of The Rab10 protein.(4)Mi R-106b-5p and Rab10 proteins can be used as biological markers for the diagnosis,treatment and prognosis of AML.
