| Objective1.To understand the relationship between H.pylori infection and related factors such as children’s age,gender and parents’education,and further analyze the high-risk factors affecting children’s H.pylori infection.2.To study the relationship between different strains of H.pylori infection and the positive expression rate of ZO-1 protein and the delta over baseline(DOB)of 13C breath test(13C-UBT),further clarify the correlation between ZO-1 protein and H.pylori infection in children,and evaluate the relationship between the type of H.pylori strain and the degree of gastric mucosal injury.Methods1.Research objectFrom June 2016 to June 2021,490 children in Qingdao were treated in the AffiliatedHospital of Qingdao University for gastrointestinal symptoms such as"nausea,vomiting,abdominal pain,abdominal distension,acid reflux and black stool".2.research method(1)The histopathological examination of gastric mucosa and 13C breath test were performed in 490 children with gastrointestinal symptoms to evaluate the infection rate of H.pylori,and the related factors affecting the infection rate of H.pylori were analyzed.(2)The content of sp-o protein in gastric mucosa of 77 children with gastric mucosal infection was measured by immunohistochemical method;The serum antibody expression of H.pylori was detected by protein chip method.According to the serum antibody typing after H.pylori infection,H.pylori was divided into three different strains;The DOB value of children was obtained by 13C-UBT,and 77 children with H.pylori infection were divided into three groups according to the DOB value.(3)SPSS 23.0 was used to process the collected data.The data was expressed in`x±s,and the counting data was expressed in percentage(%).Thec~2test was used for the comparison between independent two groups,multiple groups of binary variables and independent multiple groups of binary variables.When the theoretical data in the contingency table was less than 5,Fisher exact probability method was used,and the difference was statistically significant with P<0.05.Results1.Results on the infection rate of H.pylori:(1)The infection rate of H.pylori in 490 children in pediatrics of our hospital due to gastrointestinal symptoms was 21.84%,including 28.08%in male children and 12.63%in female children.There was significant difference in the infection rate of H.pylori between men and women(c~2=16.51,P<0.01);(2)The infection rate of H.pylori was different in different age groups,and the infection rate of H.pylori increased with age.(3)In the study of the effect of parents’education on the infection rate of H.pylori inchildren,it was found that the infection rate of H.pylori in children with high parents’education was significantly lower than that in children with low parents’education,and the difference was statistically significant(c~2=6.27,P<0.01);(4)A total of 107 children with H.pylori infection were diagnosed in this study.The pathological analysis of the gastric mucosa of the diagnosed children showed that among the children with H.pylori infection with gastrointestinal symptoms,the degree of gastric mucosa inflammation was mild,moderate and severe,accounting for 3.73%,91.59%and4.68%,respectively;The degree of inflammatory activity was mild(46.73%),moderate(50.47%)and severe(2.80%).2.Results on the relationship between different strains of H.pylori and ZO-1 protein positive expression rate and DOB value:(1)Relationship between the H.pylori different strains and ZO-1 protein positive expression rate:The positive expression rate of type I H.pylori ZO-1 protein was 15.22%,that of intermediate H.pylori ZO-1 protein was 20.00%,and that of type II H.pylori ZO-1 protein was 56.25%.There was significant difference in the positive expression rate of ZO-1 protein between type I and type II strains(c~2=9.74,P<0.01);There was no significant difference in the positive expression rate of ZO-1 protein between type I strain and intermediate strain,and between type II strain and intermediate strain(c~2=0.19,P=0.70;c~2=4.29,P=0.07).(2)The proportion of DOB values in different groups in type I H.pylori:group 1accounted for 23.91%,group 2 accounted for 32.61%,and group 3 accounted for 43.48%;The proportion of DOB value in different groups in typeⅡH.pylori:group 1 accounted for 18.75%,group 2 accounted for 62.50%,and group 3 accounted for 18.75%;The proportion of DOB values in different groups of intermediate H.pylori:group 1accounted for 46.67%,group 2 accounted for 33.33%,and group 3 accounted for 20.00%.There was no significant difference in DOB value between different strains of H.pylori(c~2=7.91,P>0.05);(3)The positive expression rate of ZO-1 protein in DOB value of different groups:group 1 accounted for 23.81%,group 2 accounted for 33.33%,and group 3 accounted for15.39%.There was no significant difference in the positive expression rate of ZO-1protein among different DOB value groups(c~2=2.36,P>0.05).Conclusions1.The H.pylori infection rate of children who were hospitalized in the Affiliated Hospital of Qingdao University due to gastrointestinal discomfort was lower than the lower limit of the total H.pylori infection rate of children in China,and the H.pylori infection rate was affected by age,gender and parents’education.2.The histopathology of gastric mucosa in children with Helicobacter pylori infection showed that the degree of inflammation was moderate>severe>mild;The proportion of severe,light and medium-sized inflammatory activity is similar,suggesting that children infected with H.pylori have atypical symptoms and nonspecific clinical manifestations,which are easy to be ignored by parents and pediatricians.3.The positive expression rate of ZO-1 protein in gastric mucosa of children was different between type I and type II H.pylori strains:type I>type II;There was no difference between type I and intermediate type and between type II and intermediate type,indicating that type I strain had stronger infectivity and greater mucosal damage.4.There was no significant correlation between DOB value and the type of H.pylori strain and the positive expression rate of ZO-1 protein.Therefore,DOB value could not be used to judge the type of H.pylori strain infected and the degree of gastric mucosal damage after H.pylori infection. |
