After the SARS epidemic in 2003,China has gradually established a relatively complete system of disease prevention and control institutions,which is composed of four levels of institutions at the national,provincial,prefecture and county levels.The main public health functions of the system are disease prevention and control,emergency response to public health emergencies,information management of epidemic situation and health-related factors,laboratory testing analysis and comments,health education and health promotion,technical guidance and application research.Among them,the county-level disease control institutions are the lowest level of this system,responsible for the specific implementation of the above public health service functions for the public.In the COVID-19 prevention and control work since 2019,the county-level disease control institutions have played an important role in technical support,but they also exposed some shortcomings and defects,including unclear status of legal subjects in the functions of public health service,unclear functions and responsibilities,imperfect management mechanism,insufficient service capabilities,and lack of coordination.On February 10,2020,General Secretary Xi Jinping stressed that We should take the construction of the national disease control system as a fundamental construction,strengthen the construction of prevention and control talents,scientific research forces and legislation at all levels,and promote the modernization of the disease control system.It is an urgent issue to explore the direction of the reform of county-level disease prevention and control institutions and how to improve the public health service capacity of the disease control institutions.Taking the disease control and prevention center of H county as the investigation point,this paper summarizes and analyzes its legal positioning,institutional nature,working mechanism,current situation of public health service capacity,existing problems and reasons in the performance of public health service functions with literature,interview,questionnaire and other research methods.By combining with the current situation of disease control system reform and based on the relevant research results and advanced experience at home and abroad,this paper explores and clarifies the functional orientation of the county-level disease control institutions,improves the public health service capacity of the county-level disease control institutions,and improves the grass-roots disease prevention and control system from the perspective of the reform of simplifying the administration and delegating power,putting the management together and optimizing service,so as to put forward practical reform directions and suggestions.Firstly,simplify the administration and delegating power,revise laws and regulations,clarify the functional orientation of the county-level disease control institutions,strengthen the disease prevention and control at the town and village level,and realize the community-based and grid management of major epidemic prevention and control,to make county-level CDCS perform their functions of public health services more smoothly;secondly,putting the management together and healthy governance:establish a long-term working mechanism and build a management system of combination of peacetime and wartime and integration of medical treatment and prevention,to make county-level CDCS perform their functions of public health services more specification;and thirdly,optimizing services and enhancing capacity;implement the project of meeting the standards for infrastructure and hardware construction of disease control institutions,strengthen the introduction and training of professional and technical personnel,and comprehensively strengthen their public health service capacity and level,to make county-level CDCS perform public health service functions more efficiently. |
