| Objective:The study aims to analyze the improvement of music therapy combined with donepezil on cognitive function and activity of daily living of patients with post-stroke cognitive impairment(PSCI),and to provide effective treatment methods for patients with post-stroke cognitive impairment.Methods:Stroke patients who were hospitalized in The Second Affiliated Hospital of Hainan Medical College from June 2020 to June 2021 were selected for cognitive function evaluation,and 120 patients of whom met the inclusion criteria were randomly divided into two groups by using the method of digital table.There were 60 cases in the control group and 60 cases in the experiment group.Both groups were treated with anti-platelet aggregation,improving circulation,regulating lipid to stabilize plaque,controlling blood pressure and glucose,routine rehabilitation exercise,symptomatic and supportive treatment.Both groups were treated with Donepezil.The experiment group was given music therapy on the basis of control group.The patients were followed up regularly,and were urged to complete the intervention training content on time for 6 months.The mini-mental state examination(MMSE),montreal cognitive assessment(MoCA)and activity of daily living(ADL)were used for assessment on PSCI patients respectively before the intervention,3months after the intervention and 6 months after the intervention.After 6 months,56 patients in the control group completed the scale evaluation(4 cases dropped out),and 57 patients in the experiment group completed the scale evaluation(3 cases dropped out).The collected data were sorted for statistical analysis.Results:1.MMSE,MoCA and ADL scores of 2 groups at different time before and after intervention were statistically significant at different time,groups and interaction between time and groups according to the results of the generalized estimating equation(GEE)(P<0.05).2.Comparison between different groups at the same time :there was no statistically significant difference in the MMSE score between the 2 groups after 3 months of intervention(P>0.05).And the MMSE score of the experiment group was significantly higher than those of the control group after 6 months of intervention,with statistically significant difference(P<0.05).ADL and MoCA scores of the experiment group were significantly higher than those of the control group after 3 and6 months of intervention,with statistically significant difference(P<0.05).3.Comparison between the same groups at different time :(1)MMSE score of 2groups after 3 months of intervention was significantly higher than those before intervention,and MMSE score of experiment group after 6 months of intervention was significantly higher than those before and after 3 months of intervention,with statistically significant difference(P<0.05).There was no significant difference in MMSE score between 3 months and 6 months of intervention in the control group(P>0.05).(2)The ADL score of 2 groups after 3 months intervention was significantly higher than that before intervention,and the ADL scores of the 2 groups after 6 months of intervention were significantly higher than those before and after 3months of intervention,with statistically significant difference(P<0.05).(3)The MoCA scores of the 2 groups after 6 months of intervention were significantly higher than those before and after 3 months of intervention,and the MoCA scores of the experiment group after 3 months of intervention were significantly higher than those before the intervention,with statistically significant difference(P<0.05).There was no significant difference in the MoCA score of the control group after 3 months of intervention and before the intervention.(P>0.05).4.Comparison of MMSE scores at different items between the two groups(1)Comparison between different groups at the same time :There were no statistically significant differences in memory,attention,count,recall ability,and language ability of patients in 2 groups after 3 months of intervention,and no statistically significant difference in recall ability of 2 groups after 6 months of intervention(P>0.05).Orientation of the experimental group after 3 months intervention and orientation,memory,attention and count,and language ability of the experiment group after 6months of intervention were significantly higher than those of the control group,with statistically significant difference(P<0.05).(2)Comparison between the same groups at different time:(1)control group:In the control group,there was no statistically significant difference in memory before and after intervention(P>0.05).Orientation,attention,count,recall ability and language ability of patients after 6 months of intervention were all higher than those before intervention,with statistically significant differences(P<0.05).And attention,count,and language ability after 3 months of intervention were significantly higher than those before intervention,with statistically significant difference(P<0.05).(2)Treatment group: Orientation,memory,attention,count,recall ability and language ability were significantly higher after 6 months of intervention than those before and3 months of intervention,and orientation,attention,count and language ability after 3months of intervention were significantly higher than those before intervention,with statistically significant difference(P<0.05).Conclusion:music therapy combined with donepezil and Donepezil treatment alone can promote the improvement of cognitive function and the level of activities of daily living of patients with PSCI in both groups,but the effect of the former is more obvious.Especially in terms of orientation,memory,attention and computation,and language ability.This suggests that music therapy combined with donepezil has a significant improvement effect on patients with PSCI.This study provides more effective treatments for patients with PSCI.and music therapy is also economical,simple and easy for patients to accept,which is worth popularizing in clinical work. |
