Objective: To evaluate the clinical efficacy of platelet-rich fibrin(PRF)assisted autologous granular fat transplantation by meta-analysis.Methods:Articles published before November 2021 on the clinical efficacy of platelet-rich fibrin-assisted autologous granular fat transplantation were searched in the Pubmed,Cochrane library,Embase,CNKI,China Biomedical Literature,Wan Fang Medical and other databases.According to the inclusion and exclusion criteria, the retrieved articles were screened, then relevant data in the article finally included in this meta-analysis is extracted, and analyzed by the Review Manager5.3 software. Results: A total of 247 articles were retrieved.According to the inclusion and exclusion criteria,12 clinical studies on PRF assisted fat transplantation were finally included, including 8 randomized controlled trials and 4 cohort studies, among them,7 papers used pure granular fat as the control group,and5 papers used platelet-rich plasma assisted fat transplantation as the control group.According to different control groups, the differences in the incidence of complications, the rate of secondary transplantation, the effective rate from doctor’s evaluation, and patient satisfaction of PRF assisted fat transplantation were analyzed respectively. The results showed that:(1)In terms of the incidence of complications,PRF assisted The fat transplantation group was lower than the simple granular fat transplantation group, and the difference was statistically significant(RR=0.35,95%CI: 0.13-0.94,P=0.04);(2)In terms of the rate of secondary transplantation, the PRF assisted fat transplantation group was lower than the simple granular fat transplantation group, the difference was statistically significant(RR=0.48,95%CI: 0.35-0.68,P <0.0001),and it was also lower than the PRP assisted fat transplantation group, the difference was statistically significant(RR=0.41,95%CI: 0.27-0.64,P<0.0001);(3)In the effective rate from doctor’s evaluation, the PRF assisted fat transplantation group was higher than the PRP fat transplantation group, and the difference was statistically significant(RR=1.18,95%CI: 1.04-1.33,P=0.010);(4)In terms of patient satisfaction, the PRF assisted fat transplantation group was higher than the simple granular fat transplantation group, and the difference was statistically significant(RR=0.1.29,95%CI:1.14-1.45,P<0.0001),but compared with the PRP assisted fat transplantation group, the difference was not statistically significant(RR=1.08,95%CI:0.96-1.21,P=0.18).Conclusion: Compared with granular fat transplantation,PRF assisted fat transplantation can reduce the Incidence of complication, the rate of secondary transplantation and patients are more satisfied with the postoperative effect.Compared with the PRP assisted fat transplantation,PRF assisted fat transplantation makes the rate of secondary transplantation more lower, and the effective rate from doctor’s evaluation more higher, but there is no significant difference in patient satisfaction between the two groups.
