Observation Of Clinical Efficacy Of Baiban Ointment Combined With Red Light In The Treatment Of Vulvar Lichen Sclerosus With Liver-kidney Yin Deficiency

Objective:To observe the clinical efficacy of Baiban ointment combined with red light in the treatment of vulvar lichen sclerosus with liver-kidney yin deficiency,and to develop a new thought and way for the treatment of vulvar lichen sclerosus by using traditional Chinese medicine.Methods:All patients in this study were admitted to the gynecological outpatient clinic of Tianjin Academy of Traditional Chinese Medicine affiliated hospital from December 2020 to October2021,and 82 vulvar lichen sclerosus patients with liver-kidney yin deficiency who met the inclusion and exclusion criteria were selected.And they were categorized at random into 41 cases as the treatment group as well as 41 cases as the control group.The treatment group was given external application of Baiban ointment combined with red light therapy,while another group was given mometasone furoate cream for external use.Three courses of treatment were followed by evaluation of clinical efficacy.4 weeks was a course of treatment.The frequency of the three courses of treatment in this study was once a day,once every two days,and twice a week.The degree of genital itching,lesion range,depigmentation,skin elasticity,TCM symptoms,vulvar quality of life index(VQLI)and generalized anxiety disorder scale(GAD-7)were evaluated before the treatment and after the end of three courses of the treatment.Observe and record adverse reactions,and follow up 3 month after leaving the group to observe the recurrence of patients.The data obtained in this study were statistically analyzed by SPSS 26.0.Results:1.Before treatment,there was no statistical difference in terms of the age,course of disease,menopause,thyroid disease,degree of genital itching,lesion range,depigmentation,skin elasticity,TCM syndrome score,VQLI and GAD-7 in the two group(P>0.05).2.In the terms of genital itching,lesion range,depigmentation,skin elasticity,the degree of the two groups were lower than before,and there had a significantly different between them(P<0.05);and the treatment group had a superior improvement comparing to another group of the degree of genital itching,lesion range,depigmentation,skin elasticity,in which the differences were found statistically(P<0.05).3.In the TCM syndrome scores of the treated patients,a reduction was observed in both groups compared with the pre-treatment period,in which the differences were found statistically(P<0.05);and the treatment group was superior to another group in terms of score improvement,all of which were statistically different(P<0.05).4.The scores of patients who were treated in the VQLI were lower than before,in which the differences were found statistically(P<0.05);and the treatment group had better impoverment in VQLI,in which the differences were found statistically(P<0.05).5.Comparing the GAD-7 scores after treatment,both group decreased than before,in which the differences were found statistically(P<0.05);and the treatment group was superior to another group of the improvement of the scores after treatment,in which the differences were found statistically(P<0.05).6.In clinical efficacy,the difference in the total effective rate between the two groups proved to be statistically significant(P<0.05),70.3% in the control group and 97.4% in the other group.7.In terms of long-term efficacy,the recurrence rate compared between the two groups differed statistically(P<0.05),which was 2.6% among the treatment group and 29.7% among the control group,and the control group had a higher rate than another group.8.In terms of safety,no significant adverse effects or changes in vital signs were observed in either group during the study,and both methods had a good safety performance.Conclusion:1.Baiban ointment combined with red light in the treatment of VLS patients with liver-kidney yin deficiency can effectively improve the degree of genital itching,lesion range,depigmentation,and skin elasticity,TCM syndromes,the quality of patients’ life,the anxiety status of patients,and the treatment group had significantly better efficacy in comparison to the other one.2.Baiban ointment combined with red light in the treatment of VLS patients with liver-kidney yin deficiency can significantly improve their clinical symptoms and signs.It has a lower recurrence rate and higher safety,and has the value of further clinical research and promotion.
