Objective:Using metabolic profiling techniques study on pathogenesis of the damp heat syndrome oral lichen planus(OLP),and then clarify the mechanism for Qingxianyin.Methods:1.20 patients with OLP who were eligible for the treatment of damp heat syndrome were treated with the herbal formula Qingxianyin,and the clinical efficacy was evaluated after 2 weeks,4 weeks,6 weeks and 8 weeks of treatment,respectively,using the visual analog scale method to assess the pain and sign scores.2.The mid-morning urine from 20 patients with OLP in the treatment group and 20 blank controls was centrifuged by high-speed frozen centrifuge,and the urine samples from both groups were tested by UPLC-MS detection technique,and the data were analyzed by Masslynx software to explore the possible pathogenesis of OLP at the metabolomic level by the overall metabolic network of the disease evidence and the changes of its potential biomarkers.3.The intervention of OLP treated with Qingxianyin was performed with the help of the urine samples of OLP patients before and after treatment with Qingxianyin were tested by UPLC-MS detection technique,and the data were analyzed by Mass Lynx software to evaluate the therapeutic effects and potential targets of Qingxianyin in terms of the changes of the overall metabolic network of the disease and its potential biomarkers.Results:1.In patients with damp heat syndrome OLP,2,4,6 and 8 weeks after treatment,the clinical symptoms and signs scores were compared with those before treatment;after 4 weeks of treatment,the clinical symptoms and signs scores of the treatment group began to show changes;after 8 weeks of treatment,the symptoms and signs scores of the treatment group showed significant changes,and the overall effective rate of the treatment group was90%(18/20),and the difference was statistically significant.2.Based on the UPLS-QTOF-MS analysis system provided accurate mass determination and secondary mass spectrometry data of biomarkers of damp heat syndrome OLP;the structures of a total of 12 variables identified in urine showed a decreasing trend compared with the blank control group.3.After 8 weeks of oral administration of Qingxianyin in OLP patients,it was determined that Qingxianyin in positive ion assay mode had a tendency to down-regulate 7 of the 12 ions that were down-regulated in urine,leaving the remaining 5 ions to continue to be down-regulated,based on the relative levels of biomarkers in urine.Conclusion:1.Qingxianyin is effective in treating damp heat syndrome OLP;2.Endogenous amino acid,sugar and sphingolipid metabolism abnormalities appear in damp heat syndrome OLP;3.Qingxianyin has good therapeutic effect on the abnormal amino acid metabolism,sugar and nucleic acid metabolism of OLP with damp heat syndrome;4.It may provide a non-invasive,patient-friendly and accurate method for diagnosis,pathogenesis and efficacy evaluation of the disease. |
