Objective:Patients with chronic kidney disease(CKD)have intestinal flora disorders,there are certain differences in the level of metabolism in the body.Uremic pruritus(UP)is a common skin manifestation in maintenance hemodialysis(MHD)patients,and there may be differences in intestinal metabolic levels.Fecal metabolomics is currently being widely used as a platform for studying metabolic changes and discovering potential biomarkers for certain diseases,so in this study we aim to find potential biomarkers and better understand their pathogenesis by studying changes in fecal metabolomics in patients with uremic pruritus.Methods:We enrolled nineteen patients on maintenance hemodialysis,including ten patients with UP and nine patients with uremic not itching.The stool samples were analyzed by liquid chromatography-mass spectrometry(LC-MS),principal component analysis(PCA)was used to distinguish the pruritus group and the non-pruritus group,and the differential metabolites of the two groups were screened by S diagram.Collate data and make statistical analysis.Results:1.Fecal metabolomics based on LC-MS revealed differences in intestinal metabolism between patients with UP and those without.Fecal metabolites showed twenty-six different metabolites associated with UP,fourteen metabolites were increased and twelve were decreased in fecal samples of UP.2.Compared with uremic patients without pruritus,fecal metabolites in patients with UP were mainly enriched in four metabolic pathways,including phosphatidylinositol signaling pathway,phosphoinositol metabolism,arachidonic acid metabolism and ascorbic acid and uronic acid metabolism pathways.Conclusion:1.We found changes in fecal metabolites in patients with UP by metabonomics,which are mainly involved in lipid metabolism,bile acid metabolism and amino acid metabolism.2.These twenty-six differential metabolites were proved to be possibly related to pruritus in this study,providing some ideas for studying potential biomarkers and related pathogenesis of UP,and providing some targets for finding therapeutic methods to solve UP.3.The metabolic pathway enrichment analysis showed that the differential metabolites between the two groups were mainly enriched in four metabolic pathways.This may explain the difference in the level of metabolites in patients with UP due to the change of metabolic pathway,and showing pruritus,which provides a certain basis for the studying of the relationship between disease and phenotype.
