Efficacy Of Steroid-soaked Gelfoam As Nasal Packing For Different Duration After Endoscopic Sinus Surgery

Objective:To investigate the therapeutic effect of gelfoam soaked with budesonide suspension as nasal packing for different duration after endoscopic sinus surgery(ESS)on patients with chronic rhinosinusitis with nasal polyps(CRSw NP).Methods:This prospective clinical study included 75 patients with chronic sinusitis with nasal polyps who had underwent bilateral endoscopic sinus surgery and completed the follow-up.After operation,the right nasal cavities of all patients were filled with gelfoam soaked with 2 mg(4 ml)budesonide suspension(steroid group),and the left nasal cavities were filled with gelfoam soaked with 4 ml normal saline(saline group).Patients were randomly divided into three groups: A,B and C,and nasal packing was performed for 2 weeks,3 weeks and 4 weeks respectively.The VAS scores and Lund-Kennedy scores of each side of nasal cavities were collected at 4 weeks,8 weeks and 12 weeks after operation,and the difference of curative effect among groups was analyzed..Results:(1)As for VAS scores,at each follow-up time after operation,there was no Statistical difference between the left and right nasal cavities in each group,or among the right nasal cavities of three groups(P>0.05).(2)As for Lund-Kennedy scores,at each follow-up time after operation,when comparing the left and right nasal cavities in each group,the steroid groups were better than the saline groups as a whole,and the difference in group C was the largest;When comparing the right nasal cavities of three groups,group C was better than group A and group B as a whole.Conclusion:The application of gelfoam soaked with hormone as nasal packing can effectively improve the objective endoscopic score and promote mucosal healing after ESS,and the longer the intervention time,the better the effect.But there was no significant improvement in score of subjective symptom.
