Objective:To analyze the related risk factors of amputation in type 2 diabetes mellitus and provide reference for clinical diagnosis and treatment.Methods:A total of 245 patients with type 2 diabetic foot who were hospitalized in our hospital from January 1,2016 to May 31,2019 were selected.245 patients with type 2diabetic foot were divided into the amputation group(n=86)and the non-amputation group(n=159).General information of all patients was collected: gender,age,occupation,residence,education,disease course,Wanger classification,etc.D-dimer,albumin,high-density lipoprotein,c-reactive protein,fibrinogen,hemoglobin,triglyceride levels,low-density lipoprotein,procalcitonin,and bilateral lower limb angiography were recorded in each group.Patients were observed for infections,amputations,and ulcers.Risk factors of amputation in diabetic foot patients were analyzed..Results:In the amputation group,there were 53 males and 33 females with an average age of(58.0±10.3)years.In the non-amputation group,there were 95 males and 64 females with an average age of(61.0±8.2)years.1.There was no significant difference in gender,diabetes course and previous medical history between amputation group and non-amputation group.There were statistical differences in age,place of residence and educational level among groups(P<0.05).2.The proportion of patients above Wagner level 3 in amputation group was87.2%,while the proportion of patients above Wagner level 3 in non-amputation group was 33.33%.The Wagner classification risk of amputation group is higher than that of non-amputation group.3.the difference between amputation group and non-amputation group is statistically significant(p<0.001)in total protein,serum albumin,high density lipoprotein cholesterol,low density lipoprotein cholesterol,procalcitonin,c-reactive protein,fibrinogen,D-dimer,total cholesterol and triglyceride,which are all related risk factors for amputation of diabetic foot patients.4.Logistic regression analysis showed that triglyceride(OR=10.323,p=0.023),high density lipoprotein cholesterol(OR=3.171,p=0.034),low density lipoprotein cholesterol(OR:=43.130,p<0.001),C-reactive protein(OR1=060,p=0.034),albumin(OR=0.750,p=0.012),D-dimer(OR= 1.376,p<0.001),and diabetic foot patients Conclusion:1.Age,residence and education level of DF patients are the influencing factors for amputation of DF patients.2.The longer the course of disease of DF patients and the higher the grade of Wanger grade are the serious influencing factors for amputation of DF patients.3.The combination of triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol,C-reactive protein,albumin and D-dimer is a risk factor for diabetic foot amputation. |
