ObjectiveThis study used an evidence-based approach to compile a summary of the best evidence for patient volume management in Heart Failure(HF).At the same time with the help of hospital information platform,the optimization of health application(APP),individualized health education module push function in patients with heart failure in implementing vulnerable period capacity management scheme based on theoretical framework IKAP,explore the solution to reduce the heart failure patients vulnerable period in the readmission rate effect,aims at providing a reference for heart failure patients vulnerable period capacity management,Promote the recovery of patients with heart failure.Methods1.Evidence-based method: PICO mode was used to construct evidence-based questions,and the whole network database was retrieved according to 6S evidence model computer system.The retrieval time was set up until December 2,2021.The quality evaluation was independently completed by 2 nursing graduate students who had received systematic evidence-based courses in the research group according to the authoritative evaluation tools corresponding to the literature types.Evidence grade evaluation adopted the Evidence Grade and Recommendation Level System of Australian JBI Evidence-based Health Care Center(2014).Evidence was classified according to the literature type of evidence extraction,and evidence strength was recommended according to the feasibility,effectiveness,suitability,clinical significance and JBI evidence grade recommendation principle.2.Intervention methods: In the control group,on the basis of routine nursing,Health Le APP was used for home capacity management in vulnerable period.Patients in the control group,on the basis of health music APP form filling,critical value to remind,health lecture hall three modules to optimize and design,further reflects the continue nursing of individualized and dynamic capacity management,in the health information project module individualized send heart failure recovery related knowledge,critical value early-warning prompt and matters needing attention,and is equipped with specialist background problem answer,Implement home capacity management in heart failure vulnerable period based on IKAP theory.Results1.Conclusion of best evidence: A total of 548 literatures were retrieved according to the retrieval strategy,and 12 literatures were finally included,including 1 clinical decision,6 guidelines,2 expert consensus,1 best evidence summary,and 2 systematic evaluation.A total of 13 best evidences were extracted from 8 aspects of heart failure vulnerability period capacity management objectives,team management,capacity assessment,water and sodium intake,urine volume management,weight management,drug management and discharge follow-up.2.Baseline results: 200 illness people with heart failure who reached the enrollment standards were selected,with 100 patients choosen in the experimental group and 100 patients allocated in the control group.During the study,6 patients in the experimental group and 8 in the control group were lost to follow-up,accounting for 7% of the study samples.Finally,94 patients were collected in the experimental group and 92 patients were collected in the control group.When comparing the baseline data of patients in the two groups,the results showed that there existed no statistical difference in baseline data between the two groups of patients(P>0.05).3.There was statistical difference in quality of life scores between the two groups in this research(P<0.05);In the vulnerable period,the research showed that statistically significant difference between the two groups in weight management ability and dry body mass compliance rate(P<0.05);In the vulnerable period,there was statistically significant difference in b-type titanium natriuretic level between the two groups(P<0.05).Conclusion1.The IKAP theory was used as the framework to successfully construct the capacity management program in the vulnerable period of heart failure patients.2.The volume management program based on IKAP theory can make the quality of life better,weight management ability and dry body mass compliance rate of patients with heart failure in vulnerable stage,reduce the level of B-type natriuretic titanium and readmission rate in vulnerable stage,and improve the use rate of application in patients with heart failure. |
