Efficacy Of Biejiajianwan Combined With Entecavir In Reversing Liver Fibrosis In Patients With Chronic Hepatitis B By VCTE

ObjectivesMonitoring the transition of liver fibrosis in patients before and after treatment by VCTE assessment,To investigate the efficacy of Biejiajianwan combined with entecavir in the treatment of chronic hepatitis B patients in reversing their liver fibrosis.MethodsThis study screened the clinical data of 239 patients with chronic hepatitis B liver fibrosis who visited the Second Affiliated Hospital of Nanchang University from January 2019 to December 2020.All patients were divided into treatment group(119people)and control group(120 people)according to different treatment plans.The control group was given basic ETV(entecavir)antiviral therapy,while the treatment group was given ETV treatment combined with Biejiajian.Pill therapy.They were divided into liver fibrosis group and liver cirrhosis group for subgroup analysis according to their initially diagnosed VCTE values.The patients with liver fibrosis were also divided into two subgroups according to the VCTE value,mild and moderate to severe fibrosis.After a certain course of treatment,the degree of VCTE liver stiffness changes in patients was examined.Within the same baseline,the efficacy of Biejiajian Pill and ETV in the treatment of HBV liver fibrosis was verified by comparing the liver elasticity value and the negative rate of HBV-DNA before and after treatment.Results(1)The LSM of the control group and the treatment group after treatment was smaller than that of before treatment,and the difference was statistically significant(P<0.0001).At the same time,the LSM of the treatment group was also significantly lower than that of the control group after treatment,and the difference was also statistically significant(P=0.0005).(2)In the liver fibrosis subgroup,the liver elasticity values of the control group and the treatment group after treatment were lower than those before treatment,and the difference was statistically significant(P<0.0001);while in the liver cirrhosis subgroup,the liver elasticity value of the control group after treatment Although the value was lower than that of the control group,the difference was not statistically significant(P=0.0975>0.05),while the liver elasticity value in the treatment group after treatment was much lower than that before treatment,and the difference was statistically significant(P<0.0001).(3)In the liver fibrosis subgroup,the number of patients with mild fibrosis who completely reversed liver fibrosis(ie,the VCTE detection value after treatment<7.3k Pa)after treatmentwas greater than that in the control group,but there was no significant difference between the two groups(χ2 =0.441 P =0.506>0.05);the number of patients with moderate to severe liver fibrosis who completely reversed liver fibrosis after treatment was much more than the control group,and the difference between the two groups was statistically significant(χ2=4.82 P =0.028<0.05).(4)When the treatment course was about 3-6 months,the negative rate of HBV-DNA in the treatment group was slightly higher than that in the control group,but the difference was not statistically significant(P>0.05);after 9 months of treatment,the negative conversion rate of patients in the treatment group reached87.4%,which was significantly higher than that of the control group(70.8%),and the difference was statistically significant(P=0.002<0.05).After 12 months of treatment,the negative conversion rate of patients in the treatment group was as high as 95%,which was better than 76.67% of the control group,and the difference was statistically significant(P<0.001).Conclusion(1)Biejiajianwan combined with ETV in the treatment of patients with chronic hepatitis B liver fibrosis can significantly improve the degree of liver fibrosis.(2)With the prolongation of the course of Biejiajianwan,the virological response rate of HBV-DNA in patients increased,and Biejiajianwan had the effect of adjuvant entecavir antiviral therapy.
