Object : Systematic evaluation of three shear wave-based elastic ultrasound(shear wave elastography,transient elastography,acoustic radiation pulse elastography)for the diagnosis of liver fibrosis and cirrhosis in patients with chronic hepatitis C,clinical value of the diagnosis and diagnosis of chronic hepatitis C disease The best solution.Method:The system searches for foreign related databases such as Cochrane library,Medline,Embase,Web of Scienc,and strictly selects the documents according to the included standards and exclusion criteria.After researching the literature and extracting relevant data and evaluating the quality,compare these to detect obvious fibers.Combined sensitivity(Se),combined specificity(Spe),and area under the SROC curve(AUC)of SF,severe fibrosis(AF),and cirrhosis,using real-time shear wave elastography using statistical analysis software(SWE),transient elastography(TE)and acoustic radiation pulse elastography(ARFI)for liver fibrosis and cirrhosis in patients with chronic hepatitis C(CHC).Result: A total of 24 patients with a total of 6017 patients in the literature,the study showed that SWE diagnosis of fibrosis combined Se,combined Spe,AUC were86.3%,87.1%,0.933,SWE diagnosis of severe fibrosis combined with Se,combined Spe were 93.0% 89.1%,the combined Se and combined Sp of SWE diagnosis of cirrhosis were 90.5% and 93.1%,respectively.The combined se,combined Spe,and AUC of TE diagnosed with obvious fibrosis were 74.9%,84.4%,and 0.868,respectively.The combined Se,combined Spe,and AUC of TE diagnosed with severe fibrosis were 75.8%,89.5%,and 0.906,respectively.The combined Se,combined Spe,and AUC were 80.4%,92.1%,and 0.926,respectively.ARFI diagnosed with fibrosis with combined Se,combined Spe,and AUC were 72.8%,81.9%,and 0.903,respectively.ARFI diagnosed severe fibrosis with Se,combined Spe,and AUC were80.1%,82.1%,and 0.944,respectively.ARFI diagnosis of cirrhosis The combined Se,combined Spe,and AUC were 86.5%,80.1%,and 0.939,respectively.Conclusion: Compared with TE and ARFI,SWE may provide the best diagnostic performance for detecting liver fibrosis and cirrhosis in patients with chronic hepatitis C.However,more prospective randomized controlled trials are needed to compare the diagnostic accuracy of these non-invasive methods for fibrosis and cirrhosis staging. |
