DSA Monitoring And Ultrasound Guided Percutaneousinjection Of Pingyangmycin Large Cystic Lymphatic Malformation Of The Recent Curative Effect And Security Of Contrast Research

Objective:Through the analysis of the short-term efficacy and complications of the treatment of venous malformations by percutaneous injection of polidocanol under DSA monitoring or ultrasound-guided,discussing the advantages and disadvantages of the two groups of treatment methods.Methods:41 patients with large cystic lymphatic malformation treated in the Department of Pediatric surgery of the first affiliated Hospital of Gannan Medical College from July 2017 to September 2021 were retrospectively collected and divided into ultrasound group(n = 26)and DSA group(n = 15)according to the mode of treatment.The treated patients returned to the outpatient clinic of our hospital for follow-up one month after treatment.if the patients did not reach the cure standard after one month,they would come to our department for diagnosis and treatment again.The clinical effects of the two groups after treatment were observed,and the changes of focus size,treatment times,follow-up time and side effects of the two groups were compared.To analyze the short-term efficacy and safety of percutaneous local injection of pingyangmycin under DSA monitoring and ultrasound guidance in the treatment of large cystic lymphatic malformation.Results:Among the 47 patients,6 children with a history of surgical treatment were excluded.41 patients who finally met the criteria were selected as subjects,including15 cases in DSA group and 26 cases in ultrasound group.The general data of patients in DSA group and ultrasound group,such as age,sex,tumor size and location of disease,were compared.The results showed that there was no statistical difference between the two groups(P>0.05).After treatment,the total effective rates of DSA group and ultrasound group were 93.33% and 61.54%,respectively.The difference of curative effect between the two groups was statistically significant(P < 0.05),and the curative effect of DSA group was better than that of ultrasound group.The lesions in DSA group and ultrasound group were significantly smaller,and the difference before and after treatment in the same group was statistically significant(P < 0.05).The size of lesions in DSA group was significantly smaller than that in ultrasound group,and the difference between the two groups was statistically significant(P < 0.05).The curative effect of DSA group was better than that of ultrasound group.The median times of treatment in DSA group and ultrasound group were 2.00[1.00,2.00] and 3.00[3.00,4.00] times,respectively.The difference between the two groups was statistically significant(P <0.05),and the treatment effect of DSA group was better than that of ultrasound group.The shortest follow-up time of DSA group was 2 months,the longest follow-up time was 3 months,and the median follow-up time was 3.00 [2.00,3.00] months.The shortest follow-up time of ultrasound group was 3 months,the longest follow-up time was 6 months,and the median follow-up time was 6.00 [5.00,6.00] months.The difference between the two groups was statistically significant(P < 0.05),and the treatment effect of DSA group was better than that of ultrasound group.In the process of diagnosis and treatment,the incidence of complications in DSA group and ultrasound group were 20.00% and 57.69% respectively,and the difference between the two groups was statistically significant(P < 0.05),and the treatment effect of DSA group was better than that of ultrasound group.The operations of both groups were successfully completed,and the median treatment cost of DSA was:8253.51[7505.42;30298.55] Yuan;The median cost of ultrasound treatment in group B was 5207.82.00 [4452.56;691.37] yuan,according to the data analysis results,there was a statistically significant difference between the two groups(P < 0.05).ConclusionLocal injection of pingyangmycin under the monitoring of DSA and under the guidance of ultrasound is effective in the treatment of large cystic LM.But DSA method is better,the curative effect is better,the safety is higher,the change of focus size is more obvious,the number of treatment is less,and the follow-up time is shorter.It reduces the psychological and social burden of children and their families,which is worthy of clinical promotion.
