【Background】: Venous malformation is the most common type of congenital vascular malformation,and its lesion range,site and symptoms are diverse,so its treatment has certain challenges.In this retrospective study,we observed the clinical effect of DSA combined with ultrasound-guided percutaneous sclerotherapy in the treatment of diffuse superficial venous malformations,compared with ultrasound-guided sclerotherapy alone,and analyzed the effective rate,cure rate and complications of the two treatment regimens.【Methods】: A total of 30 patients with venous malformation who met the inclusion criteria and were admitted to the Department of Plastic surgery and Burn in the First Affiliated Hospital of Bengbu Medical College from January 2012 to December 2020 were divided into the observation group(11 patients were treated with ultrasound combined with DSA-guided sclerotherapy).Control group(sclerotherapy under ultrasound guidance alone)19 cases.The clinical effects of the two groups were compared,that is,the reduced scope of lesions detected by color ultrasound and postoperative adverse reactions.【Results】: The observation group of 11 cases,6 cases were cured,2 cases were obviously relieved,3 cases were partially relieved,0 cases were ineffective,the effective rate was 100.00%,and each case was injected 1.37 times on average.In the control group of 19 cases,3 cases were cured,10 cases were obviously relieved,5cases were partially relieved,and 1 case was ineffective.The total effective rate was94.7%(18/19),with an average injection rate of 2.40 times.There was no statistically significant difference in response rate between the two groups(Fisher’s exact test,P=0.999).The cure rate of the observation group was 54.55%(6/11),and that of the control group was 15.79%(3/19).The difference in the cure rate between the two groups was statistically significant(Fisher’s exact test,P=0.042).The main adverse reactions were local soft tissue swelling and pain.After local ice treatment,improvement of microcirculation,intravenous dexamethasone and other symptomatic treatments,the discomfort symptoms were relieved.There were no life-threatening complications such as pulmonary embolism or cardio-cerebrovascular accident,and no gross hematuria or hemoglobinuria.【Conclusion】: DSA combined with ultrasound-guided sclerotherapy is a safe and effective treatment for diffuse superficial venous malformation.Compared with ultrasound-guided therapy alone,the cure rate is higher,but the long-term effect evaluation needs a large sample of clinical observation. |
