Objective1.To study the relationships of TCM syndromes distribution with clinical features of acute myeloid leukemia(AML);2.To detect the positive rate of gene methylation of Dickkopf-3(Dkk-3)in bone marrow cells of AML patients,and further study relationship between distribution of TCM syndromes of AML and the gene methylation of Dkk-3,aiming to explore the roles of DNA methylation in the occurrence and development of AML and provide a reference for the study of TCM syndromes and biological markers of AML.Methods:1.Related data of clinical characteristics of 93 newly diagnosed AML(non-M3)patients were collected,including gender,age,FAB typing,proportion of primitive cells in bone marrow,blood routine(WBC,HGB,PLT),and mutations of methylation related genes(TET2 and DNMT3A),and diseases of all patients were classified into different TCM syndromes,and the relations of the above clinical characteristics with TCM syndromes were explored.SPSS 26.0 software was used for statistical analysis of data results.2.A total of 45 patients newly diagnosed with AML(non-M3)were enrolled as case group and 21 patients with non-malignant hematologic disease were enrolled as control group.DNA was routinely extracted and treated with bisulfite after bone marrow were extracted from both groups as samples,and then the target sequence was amplified via PCR.The positive rates of Dkk-3 gene methylation in the two groups were detected by Pyrosequencing method,and the rules of distribution of TCM syndromes and Dkk-3 gene methylation were analyzed.Statistical analysis of data results obtained was performed using SPSS 26.0software.Results:1.Of 93 AML patients,there were no statistical significances in distribution of TCM syndrome compared with gender,FAB typing(P>0.05);the distribution of TCM syndrome showed statistically significant differences compared with age,proportion of primitive cells in bone marrow,routine examination of blood indexes,gene mutations of TET2 and DNMT3 A genes(P<0.05).2.The positive rate of Dkk-3 gene methylation of the case group was 51.1%,but Dkk-3 gene methylation didn’t occur in the control group.The positive rate of Dkk-3 gene methylation between the case group and the control group showed a significant difference(P<0.01),while no significant differences were found in age and gender(P>0.05).The distribution of TCM syndromes in the case group showed no correlation with gender(P>0.05),but was correlated with age(P<0.05).The positive rates of Dkk-3 methylation showed significant differences in patients with different TCM syndromes in the case group(P<0.05),and had significant differences in those with excess syndrome(positive rate of 73%)compared with those with deficiency syndrome(positive rate of 21%)(P<0.01).Conclusions:1.Compared with patients with syndromes of deficiency of both qi and yin and blood stasis and phlegm accumulation,those with exuberance of toxic heat have younger age,higher percentage of primitive cells in bone marrow,and higher mutation rates of TET2 and DNMT3 A genes.The WBC counts in patients with exuberance of toxic heat and accumulation of blood stasis and phlegm are higher than that in those with qi and yin deficiency.Patients with accumulation of blood stasis and phlegm have higher HGB and PLT than that those with exuberance of toxic heat and deficiency of both qi and yin.2.AML patients have higher methylation rate of Dkk-3 gene,but those with non-malignant hematologic diseases are found to be free from methylation.The distribution of TCM syndromes in AML patients is related to age,and proportion of exuberance of toxic heat is lowest,while qi and yin deficiency is the highest.The methylation rates of Dkk-3 gene show significant difference among different TCM syndromes,it is higher in blood stasis and phlegm accumulation syndrome and exuberance of toxic heat syndrome compared with qi and yin deficiency syndrome,and is higher in excess syndrome compared with those with deficiency syndrome.
