Clinical Observation On Therapeutic Effect Of Ziyin Qianyang Acupuncture Prescription Of Dysphagia In Parkinson’s Disease With Yin-deficiency And Wind-movemen

Objective:By observing the efficacy of Ziyin Qianyang acupuncture prescription in treating patients with dysphagia in Parkinson’s disease(PD)with yin-deficiency and wind-movemen,we clarified the therapeutic effect of acupuncture in patients with PD combined with dysphagia,and initially explored its kinematic mechanism affecting the swallowing temporality and coordination of dysphagia in Parkinson’s disease to provide new ideas for clinical treatment.Methods:84 patients who met the inclusion criteria of Yin-deficiency and wind-movement Parkinson’s disease with dysphagia were randomly divided into a control group and a observation group,each group had 42 cases.Both groups were given conventional basic treatment of Parkinson’s disease and swallowing rehabilitation training,the control group was given neuromuscular electrical stimulation(NMSE),and the observation group was treated with Ziyin Qianyang acupuncture prescription.Both group were treated once a week from Monday to Saturday,30 minutes per time,a total of 4 weeks.Changes of standardized swallowing assessment(SSA),penetration-aspiration scale(PAS),oral transit time(OTT),pharyngeal delay time(PDT),pharyngeal transit time(PTT),serum prealbumin(PA)and TCM symptom score were observed before and after 4 weeks of treatment,the curative effect of the two groups was evaluated by the integral change of TCM symptom score.The data were analyzed by SPSS 23.0.Results:A total of 84 cases were included in this trial,80 cases completed the clinical trial observation,4 cases did not complete,2 cases fell off in the observation group,1 case was excluded and 1 case fell off in the control group.1.There was no significant difference in basic data and objective indexes before treatment between the two groups(P>0.05).2.Objective indicators:(1)Scale evaluation:After treatment,SSA and PAS were improved in both groups(P<0.01),there was significant difference between the observation group and the control group(P<0.01).(2)Time parameter in video fluoroscopic swallowing study:After treatment,the OTT,PDT and PTT of the two groups were improved(P<0.01),there was significant difference between the observation group and the control group(P<0.01).(3)PA:After treatment,serum prealbumin in both groups was higher than that before treatment(P<0.01),and there was significant difference between observation group and control group(P<0.01).(4)TCM symptom score:After treatment,the TCM symptom score of the two groups was higher than that before treatment(P<0.01).There was significant difference between the observation group and the control group(P<0.05).3.Incidence of pneumonia:After 4 weeks of treatment,the incidence of aspiration pneumonia was 15% in the observation group and 35% in the control group(P<0.05).4.Overall clinical effect:After 4 weeks of treatment,the total effective rates of the observation group and the control group were 87.5% and 70% respectively,with statistical significance(P<0.01).Conclusions:Both Ziyin Qianyang acupuncture and NMES can improve the swallowing function,nutritional status and TCM syndrome scores of patients with dysphagia in PD with yin-deficiency and wind-movemen,but the degree of improvement is better under Zi Yin Qian Yang acupuncture prescription treatment,especially improve the start-up and timing of swallowing in PD patients,so as to reduce leakage and aspiration,and improve the effectiveness and safety of swallowing,it is worthy of clinical application.
