Study On The Correlation Between Anxiety And Depression In Different Subtypes Of Gastroesophageal Reflux Disease And Syndrome Element Characteristics

ObjectiveGastroesophageal Reflux Disease(GERD)is a common clinical disease.As the disease progresses,GERD patients are often accompanied by anxiety,depression and other mental states.These psychological states also influence the development of the disease.Therefore,through syndrome differentiation,the correlation of anxiety,depression and syndromes of the different subtypes of GERD--Non-erosive reflux disease(NERD)and Reflux esophagitis(RE)are explored and compared,to provide new ideas for TCM prevention and treatment of GERD population and theoretical guidance for TCM clinical syndrome differentiation.MethodsIn this study,150 patients admitted to the Second People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from January 2021 to February 2022 were collected.Through face-to-face questionnaire,collecting basic information of patients and four diagnostic information,and assisting patients to evaluate Hamilton rating scale for anxiety(HAMA)and Hamilton depression rating scale(HAMD).The four diagnostic information was input into the syndrome element differentiation software to obtain the syndrome element score,so as to analyze the correlation between anxiety and depression of different subtypes of GERD and syndrome elements.Results1.The pathological syndromes of GERD group were mainly stomach,liver,spleen,and the primary pathogenic factors were qi stagnation,heat,yin deficiency.The pathological syndromes of GERD with anxiety group were mainly liver,stomach,mind,and the primary pathogenic factors were yang hyperactivity,qi stagnation,yin deficiency.The pathological syndromes of GERD with depression group were mainly liver,stomach,spleen,and the primary pathogenic factors were qi stagnation,yang deficiency,qi deficiency.The pathological syndromes of NERD with anxiety group were mainly liver,mind,stomach,and the primary pathogenic factors were yang hyperactivity,qi stagnation,yin deficiency.The primary pathological syndromes of NERD with depression group were liver,stomach,spleen,and the primary pathogenic factors were qi stagnation,yang deficiency,qi deficiency.The primary pathological syndromes of RE with anxiety group were liver,stomach,mind,and the primary pathogenic factors were qi stagnation,yang hyperactivity,heat.The primary pathological syndromes of RE with depression group were liver,stomach,mind,and the primary pathogenic factors were qi stagnation,qi deficiency,yang deficiency.2.While comparing the characters of syndrome elements of each group,the ranked distribution between NERD with anxiety group and NERD without anxiety and depression group had statistical differences in characters of syndrome elements of liver,mind,yang hyperactivity,qi stagnation and yin deficiency.The ranked distribution between RE with anxiety group and RE without anxiety and depression group had statistical differences in characters of syndrome elements of liver,mind,yang hyperactivity,yin deficiency and blood stasis.The ranked distribution between NERD with depression group and NERD without anxiety and depression group had statistical differences in characters of syndrome elements of liver and yang deficiency.The ranked distribution between RE with depression group and RE without anxiety and depression group had statistical differences in characters of syndrome elements of liver,mind,qi deficiency,yang deficiency and blood deficiency.The ranked distribution between NERD with anxiety group and RE with anxiety group had statistical difference in characters of syndrome elements of yin deficiency.There was no statistical difference in characters of syndrome elements distribution between NERD with anxiety group and RE with anxiety group.Conclusion1.The mainly pathological syndromes of GERD were stomach,liver,spleen,and the primary pathogenic factors were qi stagnation,heat,yin deficiency.2.The primary pathological syndromes of NERD with anxiety and RE with anxiety were liver,mind,stomach,and the pathogenic factors were intermingled deficiency and excess,most of them were solid syndrome.The ranked distribution between NERD with anxiety and RE with anxiety had statistical difference in characters of syndrome elements of yin deficiency,it is suggested that the treatment should pay attention to nourishing yin.3.The symptoms of NERD with depression and RE with depression were mainly liver,stomach,spleen and mind,and the symptoms were mixed with deficiency and solid,mostly deficiency syndrome.
