Study On Tcm Syndrome Type Distribution Of Knee Osteoarthritis In Fujian Based On Data Mining

Objective:Through the method of data mining,the distribution law of TCM syndrome types of knee osteoarthritis in Fujian area and the relationship with age,gender,BMI index,imaging performance provide certain reference basis for its clinical application.Methods : The general information and TCM symptoms of 156 patients with knee osteoarthritis in the department of Joint Surgery of Fuzhou Second Hospital of Fujian province were collected by questionnaire.Collect the questionnaire data and input it into Excel database.The frequency and frequency of each symptom were calculated.SPSS 21.0 software was used for cluster analysis of symptoms with frequency greater than 10% to form symptom classification groups.Then the syndrome elements measurement method in Syndrome Differentiation was used to weight the symptoms of each patient to obtain the diagnosis name of the syndrome elements of the patient.The syndrome elements with a frequency greater than 10% were again summarized and imported into SPSS 21.0 for cluster analysis to form the syndrome elements classification group.Syndromes and syndrome elements were compared and analyzed,and syndromes were diagnosed and described according to the TCM diagnostic and therapeutic criteria(ZY/T001.1-94).The Apriori algorithm in SPSS Molder18.0 was used to analyze association rules of symptoms of each syndrome type.Then the frequency statistics of TCM syndrome types were carried out by SPSS 21.0.SPSS Molder18.0 software was used to draw the visual network map of TCM syndrome type,gender,age,BMI and region.Finally,the research report on the distribution law of TCM syndrome type is completed through demonstration.Results:A total of 156 cases meeting the inclusion criteria were collected.There were 53 males and 103 females,aged 42-91 years,with an average age of 66.86±8.73.By cl uster analysis,patients with knee osteoarthritis were divided into 5 categories of sympt oms and 4 categories of syndrome elements.By comparing the clustering results,the th ree syndrome types were named as kidney deficiency and marrow deficiency,blood st asis and Yang deficiency and cold coagulation based on the diagnosis and efficacy sta ndard of TCM diseases and syndromes(ZY/T001.1-94).There were 56 cases of kidney deficiency and marrow deficiency,44 cases of blood stasis and 56 cases of Yang def iciency and cold coagulation.According to association rule analysis,the support degree between pulse-sinking and lumbar and knee acerbity was the highest among kidney d eficiency and marrow deficiency syndrome.The symptom of pulse sinking is often asso ciated with the frequency of nocturnal urination,weak tongue and soreness of waist a nd knee.In blood stasis block syndrome,the support degree between fixed pain and ti ngling symptom was the highest.Fixed pain,tingling,and numbness are interrelated an d often concomitant.In Yang deficiency cold coagulation syndrome,the support degree between limb temperature and moss white symptom was the highest.Cold limbs,cold jo ints,moss white three symptoms often accompany.By frequency analysis and Chi-square test,there was no significant correlation between gender and syndrome type distributi on in this study(χ~2=4.15,P>0.05),and age(χ~2=46.89,P<0.01),BMI(χ~2=10.45,P<0.05),perm anent residence(χ~2=20.37,P<0.05),imaging findings(χ~2=38.99,P<0.05)has certain correla tion.Conclusions:Through cluster analysis and syndrome element analysis,the TCM syndrome types of knee osteoarthritis in Fujian were named as: kidney deficiency and marrow deficiency,blood stasis and stagnation,Yang deficiency and cold coagulation,all of which were dominated by deficiency.According to the characteristics of the three syndrome types and syndrome elements,the TCM treatment of knee osteoarthritis can be determined as nourishing kidney and warming Yang,activating blood and clearing collaterals,nourishing Yin and clearing heat.The TCM syndrome type of knee osteoarthritis has certain correlation with age,BMI,permanent residence and imaging manifestations.More than 70-year-old patients with syndrome differentiation for the kidney deficiency and marrow deficiency type more than other age groups,the number of "overweight" and "obesity" group and the strongest,the correlation of blood stasis type significantly more than the hills for coastal local patients,patients with coastal areas more show the kidney deficiency and marrow deficiency and Yang deficiency and cold coagulation,hilly region more performance for patients with blood stasis type.K-l grade IV was the most frequent and most correlated type of kidney deficiency and marrow deficiency.
