Objective:This study was conducted to compare the levels of Lp-PLA2,MHR between the premature coronary heart disease(PCHD)group and the normal control(CON)group,as well as the correlation between the Lp-PLA2 MHR and the severity of coronary artery disease in PCHD patients.Methods:A total of 242 patients with suspected coronary heart disease,such as chest tightness,chest pain and shortness of breath,who were hospitalized in the Department of Cardiology,Dongguan Cardiovascular and Cerebrovascular Disease Hospital affiliated to Yan’an University from August 2020 to January 2022.The males with the age of≤55 years old and females with the age of≤65 years old,including 125 males aged 46.31±7.22 years old,117 females,aged 54.38±6.38 years old.The hospitalized patients received coronary angiography or coronary artery CTA examination.After the completion of auxiliary examination,the patients were divided into PCHD group(137 cases)and CON group(105cases)according to the examination results.The patients with premature coronary heart disease were regrouped according to their symptoms,signs and auxiliary examinations,including 58 cases of stable angina,43 instances of unstable angina and 36 instances of acute myocardial infarction.Then as indicated by the coronary vein GENSINI scores,the patients with PCHD were isolated into low risk group(66 cases,scores≤40)and high risk group(71 cases,scores>40).After hospitalization,the general data of all patients were collected in our hospital system,mainly including Gender,age,smoking history,history of carotid atherosclerosis,history of diabetes,history of hypertension,etc.Height and weight were recorded,and body mass index(BMI)was calculated according to the formula.Record the general data and test indexes in time and save them in computer Excel.After admission,all patients were selected for emergency coronary CTA or appropriate coronary angiography according to their specific conditions(e.g.,previous history,symptoms,signs,electrocardiogram,etc.).The angiography data were reviewed in time,and GENSINI scores were performed and recorded.Later,patients with PCHD were divided into low risk group and high risk group according to GENSINI scores;the utilization rate of counting data was expressed accordingly,the chi-square test was used for comparison between groups.When measurement data pass normality test and homogeneity of variance test,the data is described by?x±s methods and studied by one-way variance methods;Lsd-t test was used to analyze data for comparison between groups.Spearman rank relationship investigation was utilized to assess the connection between’s Lp-PLA2,MHR and GENSINI scores.Results:1.Compared with the control group,the prevalence of male smoking,diabetes and hypertension in the general data of PCHD patients was significantly higher.The levels of IMA,TC,HDL-C,LDL-C,APo AI,Lp(a),Lp-PLA2 and MHR in laboratory were higher than those in control group,there were statistically significant differences between the two groups(P<0.05).Gensini scores were performed for coronary artery lesions in the PCHD group,and the patients with premature coronary heart disease were separated into high level and low level groups as per the score.2.Regrouping of PCHD was found that Lp-PLA2 and MHR in UAP group and AMI group were significantly higher than those in SAP group(P<0.05).MHR in AMI group was significantly higher than that in UAP group(P<0.05).3.Pearson rank correlation analysis was performed on Lp-PLA2、MHR and GENSINI scores respectively in the PCHD.Lp-PLA2、MHR was positively correlated with GENSINI scores(r_s=0.637,P<0.001;r_s=0.512,P<0.001).4.When the critical value of Lp-PLA2 was 238 ng/m L,the Yoden index reached the maximum value of 0.527,Its diagnostic value for PCHD high GENSINI scores reached the maximum,with the sensitivity of 64.8%and specificity of 87.9%.When MHR critical value=1.135,the Yoden index reaches the maximum value:0.392,the sensitivity and specificity of PCHD were 63.4%and 75.8%,respectively.Conclusion:1.Lp-PLA2 and MHR in PCHD group were higher than those in CON group.2.Lp-PLA2 and MHR are emphatically associated with the seriousness of coronary stenosis in PCHD,which have good diagnostic value for high GENSINI scores of PCHD. |
