| Objectives:To analyze the HPV infection in gynecological patients of Yan’an University Affiliated Hospital of Yan’an University of different ages and cervical lesions,and to analyze the effect of multiple infection with HR-HPV 16/18 on the severity of cervical lesions compared with single infection;The effect of multiple infections of different types on the prognosis of cervical cancer is stronger than that of single infection.Studying the influence of multiple HR-HPV infection on cervical lesions and prognosis of cervical cancer in our hospital can preliminarily reflect the distribution characteristics of HPV infection in this area,and guide the anti-HPV treatment and targeted vaccination of cervical lesions and cervical cancer patients in this area.It also provides a clinical basis for later larger-scale studies.Methods: From January 2020 to December 2020,a total of 1058 cases who were admitted to the Department of Gynecology of Yan’an University Affiliated Hospital for cervical pre-cancer screening including HPV detection and met the inclusion and exclusion criteria due to related discomfort symptoms were selected.The distribution characteristics of segmental HR-HPV;among them,648 patients underwent cervical biopsy.The distribution characteristics of HR-HPV infection in different cervical lesions were analyzed,and HR-HPV-negative patients were excluded,and HPV16,18 or other single infection was divided into HR-HPV single infection group,including HPV16 type combined with 18 type,HPV16 type combined with other types or HPV18 type combined with other types was divided into HR-HPV multiple infection group,and analyzed by SPSS 25.0 software by chi-square test or Fisher’s exact probability method Whether the multiple infection group containing HR-HPV16/18 type affects the severity of cervical lesions compared with the single infection group,risk factor analysis was performed for those with statistically positive results,and the OR value(95%CI)was used to calculate The strength of the association between multiple infections of type 18 and cervical lesions at all levels;a total of 123 cervical cancer patients who were initially diagnosed and treated at Yan’an University Affiliated Hospital from January 2014 to January 2017,underwent HPV testing,and met the inclusion and exclusion criteria were selected.The general data,pathological data and related treatment conditions of the patients were collected,including: HR-HPV infection,age,stage,degree of differentiation,depth of myometrial invasion,vascular invasion,lymph node metastasis,treatment status and disease-free survival time,total Survival time,etc.,they were divided into multiple infection group containing HR-HPV16/18 type and single infection group,and SPSS 25.0 software was used for chi-square test analysis or Fisher exact probability method to analyze multiple infection containing HR-HPV16/18 type.The effect of single infection on prognosis of cervical cancer.Results: 1.The HR-HPV infection rate in this hospital was 29.6%,among which the number of infected persons aged 40-60 was the most,mainly HR-HPV single infection,mainly HPV16 and other HPV types.2.Except for HISL,which is mainly HR-HPV multiple infection,other cervical lesions at all levels are mainly single infection.HR-HPV single infection is mainly HPV16 type and other HPV type infection,and multiple infection is HPV16 type combined with other HPV type.Infection predominates.3.The multiple infection group containing HR-HPV16/18 increased the risk of cervical LSIL and HSIL,but not the risk of SCC.The ratio of multi-infection group containing HR-HPV16/18 type to single-infection group in LSIL group and HSIL group was 1.036,1.656 times of NILM,95%CI [1.004-1.069],[1.420-1.932].4.HR-HPV in cervical cancer patients in terms of age,stage,degree of differentiation,depth of muscle invasion,vascular invasion,lymph node metastasis,and adjuvant therapy are mainly single infection of HPV16 type and other HPV types,including HR-HPV16 There were no differences in age,stage,degree of differentiation,depth of myometrial invasion,vascular invasion,lymph node metastasis,and treatment for cervical cancer patients with multiple infection of /18 type compared with single infection;The 3-year DFS of the single infection group was significantly different(P=0.004<0.05);the 5-year OS of the multiple infection group containing HR-HPV16/18 was significantly different than that of the single infection group(P=0.028<0.05).Conclusion: 1.In cervical HPV detection,special attention should be paid to HPV16 and other types of infection in people aged 40-60.2.In all cervical lesions including chronic cervicitis,attention should be paid to the infection of HR-HPV,especially the infection of HPV16 and other types of HPV.In HSIL,special attention should be paid to the infection of HR-HPV16 combined with other types.3.The multi-infection group containing HR-HPV16/18 increased the risk of LSIL and HSIL,but did not increase the risk of SCC.4.Multiple infection with HR-HPV16/18 type was a worse prognostic factor for cervical cancer than single infection. |
