| Objective: To study the clinical efficacy and safety of Zhuang medicine thread moxibustion combined with Xiaoyan Zhiyang formula in the treatment of vulvar and perianal pruritus.Methods: 120 patients with vulvar and perianal pruritus who met the inclusion criteria were randomly divided into three groups.40 patients in group A were treated with Zhuang medicine thread moxibustion(moxibustion of local skin lesions and acupoints)combined with anti-inflammatory and antipruritic formula;40 cases in group B were treated with Xiaoyan Zhiyang decoction;40 cases in the control group were treated with tacrolimus ointment.The course of treatment of the three groups was 4 weeks.The clinical efficacy was evaluated by using the scoring table of pruritus symptoms and signs;The dermatology life quality index(DLQI)was used to evaluate the quality of life;Adverse reactions during treatment were recorded;One month after treatment.Through the statistical analysis of research data,this paper expounds the clinical efficacy and advantages of Zhuang medicine thread moxibustion combined with Xiaoyan Zhiyang formula in the treatment of vulvar and perianal pruritus.Results: A total of 120 cases were included,and 120 cases were finally treated,including 40 cases in group A,40 cases in group B and 40 cases in the control group.The basic data and clinical symptoms of the three groups before treatment were analyzed.There was no significant difference in gender,age and course of disease among the three groups(P >0.05),Before treatment,there was no significant difference in the scores of pruritus symptoms and signs(pruritus degree,frequency of pruritus,duration of pruritus,effect of pruritus on sleep,surrounding skin conditions)and dermatology life quality index(DLQI)among the three groups(P>0.05).The statistical analysis results of data after treatment are as follows:(1)The overall effective rate of Zhuang medicine thread moxibustion combined with Xiaoyan Zhiyang formula group was 95%;The overall effective rate of Xiaoyan Zhiyang external washing formula group was 85%;The overall effective rate of tacrolimus ointment for external use was 87.5%.There was significant difference between treatment group A,treatment group B and control group(P<0.05),and there was no significant difference between treatment group B and control group(P >0.05),indicating that the curative effect of treatment group A was better than that of the other two groups.(2)The clinical symptoms of treatment group A,treatment group B and control group were compared at each treatment time point(P<0.05).(3)There was significant difference in the score of pruritus symptoms and signs(pruritus degree,frequency of pruritus,duration of pruritus,effect of pruritus on sleep and surrounding skin conditions)and total score between treatment group A,treatment group B and control group(P<0.05).There was significant difference in the degree of pruritus between treatment group B and control group after treatment(P<0.05).There was no significant difference in the frequency of pruritus,duration of pruritus,the impact of pruritus on sleep and the improvement of surrounding skin conditions(P>0.05).(4)DLQI comparison: before treatment,the average DLQI score of group A was(20.25± 3.60),that of group B was(19.58 ± 3.17),and that of the control group was(20.08 ± 3.83).There was no significant difference in DLQI between the three groups before treatment(P >0.05).After treatment,the average DLQI score of treatment group A was(5.38 ± 2.40),the average DLQI score of treatment group B was(8.43 ± 2.67),and the average DLQI score of control group was(8.60 ± 2.10).There were differences in the DLQI scores of the three groups after treatment(P<0.05),and the DLQI score of treatment group A was significantly lower than that of treatment group B and control group.There was no significant difference between treatment group B and control group(P>0.05).(5)Comparison of adverse reactions: there were no adverse reactions in treatment group A and treatment group B,and the incidence of adverse reactions in the control group was 17.5%.The common side effects of the control group were burning and spicy pain on the skin after initial use.(6)Comparison of recurrence rate: one month after the end of treatment,follow up the patients who are effective in the three groups,and judge whether they have recurrence by scoring the clinical symptoms again.Compare the recurrence rate of the three groups.The recurrence rate of group A is lower than that of group B and control group(P<0.05).The recurrence rate of Zhuang medicine thread moxibustion combined with Xiaoyan Zhiyang recipe was 13.16%;The recurrence rate of Xiaoyan Zhiyang recipe was 50.0%;The recurrence rate of topical tacrolimus ointment was 51.43%.There was significant difference between treatment group A,treatment group B and control group(P<0.05),and there was no significant difference between treatment group B and control group(P >0.05),indicating that the recurrence rate of treatment group A was lower than that of the other two groups.Conclusion:(1)External washing of anti-inflammatory and antipruritic formula can effectively treat vulvar and perianal pruritus,and the curative effect is equivalent to that of external tacrolimus.(2)The curative effect of Zhuang medicine thread moxibustion combined with Xiaoyan Zhiyang formula in the treatment of vulvar and perianal pruritus is significantly better than that of external washing or tacrolimus.(3)Zhuang medicine thread moxibustion combined with anti-inflammatory and antipruritic formula has less adverse reactions and high safety,which can be popularized in clinic. |
