| Objective: Osteonecrosis of the femoral head is clinically multiple and difficult to treat.The disability rate is high,which seriously affects the life of patients and increases the economic burden.If the operation is performed,there are related risks and renovation accidents.In the early application of traditional Chinese medicine intervention effect is better,this study based on the clinical curative effect observation.Methods: The members included in the study were all in line with the standards of the Chinese Medical Orthopedics Branch and the dialectical standards of traditional Chinese medicine,and there was no staging difference.A total of 94 patients were divided into the observation group,the control group A and the control group B according to the patients ’inclination to see a doctor(the number of patients was 34,30,30 in turn).The observation group was treated with Henggu,alendronate sodium and chemical prescriptions.The scheme of group A was the above-mentioned bisphosphonates combined with non-steroidal drugs.The scheme of group B was Osteoking Mixture combined with non-steroidal drugs.The final overall comprehensive therapeutic effect was observed by comparing Harris,BADL,VAS,clinical efficacy of traditional Chinese medicine and imaging score.The network pharmacology and molecular docking technology were also explored.Results: The final results showed that the overall effective rate of the observation group was 97.06 %,and the control group A and B were 90 % and 80 %,respectively.According to the standard judgment of the four stages of cure,improvement,effective and ineffective,the statistical difference was significant(P < 0.01),and the final imaging improvement results showed that the difference among the three groups was also significant(P < 0.01),and the observation group was the best.Based on network pharmacology and molecular docking technology,the scheme of the observation group was topologically constructed.The results showed that there were 259 effective components in the prescription,which might play a role in DNA transcription,RNA polymerization and apoptosis by 145 targets through the regulation of NF-κB,MAPK,Fox O,PI3 K-Akt,m TOR,Wnt,HIF-1 and VEGF signaling pathways.The molecular docking results further suggested that it might mediate bone reconstruction by interfering with MAPK and EGF family members to regulate angiogenesis in necrotic areas.Conclusions:The early stage of ONFH surrounding collapse is better in young and middle-aged patients with male-dominated history of alcoholism.The combination of traditional Chinese and Western medicine treatment is more conducive to achieving relatively stable curative effect in the relatively long-term joint function,pain degree and imaging improvement within one year.However,the addition of non-steroidal drugs in the early stage of the disease has better short-term rapid remission effect than traditional Chinese medicine.The intervention of traditional Chinese medicine in the combination treatment may mainly affect MAPK and EGF family members and related pathways. |
