Objective: This study investigated the effect of Peibao Yusi decoction(PYD)on the outcome of frozen-thawed embryo transfer(FET)in patients with thin endometrium due to kidney deficiency and blood stasis,and the possible mechanism of its effect.It is expected to provide new ideas and methods to improve FET outcomes in patients with thin endometrium with kidney deficiency and blood stasis.Methods: The clinical study used a prospective research method to analyse the clinical data of 70 patients with thin endometrium due to kidney deficiency and blood stasis.There were 33 cases in the experimental group(Chinese medicine PYD with estrogen treatment)and 37 cases in the control group(estrogen treatment alone).All patients underwent freeze-thaw embryo transfer using the alternative cycle endo-preparation protocol.The test group started to take the self-prepared Chinese herbal formula PYD on the 2nd to 5th day of menstruation after embryo freezing,one dose daily,divided between morning and evening,until the day of transfer.1.Comparison of FET outcomes,evidence score of kidney deficiency and blood stasis on the day of transplantation and ultrasound-related indices.2.To compare the changes in the symptom scores and ultrasound-related indexes of kidney deficiency and blood stasis evidence before and after treatment between the two groups.Results: 1.In terms of the efficacy of the Chinese medical evidence,after the treatment,both patients in the test group and the control group had lower scores of the evidence of kidney deficiency and blood stasis than before the treatment,and the difference was statistically significant(P < 0.01).Patients in the test group had a significantly lower score of kidney deficiency and blood stasis symptoms than the control group after treatment,and the difference was statistically significant(P < 0.01).The total effective rate of clinical symptoms in the test group was significantly higher than that in the control group,and the difference was statistically significant(P < 0.01).2.In terms of three-dimensional ultrasound measurements,the endometrial blood flow grading and the subendometrial pulsatility index of the patients in the test group improved after treatment compared to that before treatment,with a statistically significant difference(P < 0.05).The endometrial thickness,endometrial volume,endometrial vascularization index,endometrial blood flow index and vascularization blood flow index in the test group improved after treatment compared with that before treatment,and the difference was statistically significant(P < 0.05).After treatment,the endometrial volume,endometrial vascularization index,endometrial blood flow index and vascularization blood flow index were positively correlated with endometrial thickness.The rest of the test groups showed a trend of improvement in all indicators,but the difference was not statistically significant.3.There was no significant difference between the test and control groups in all indicators of clinical outcome(P > 0.05),but the embryo implantation rate,clinical pregnancy rate and sustained pregnancy rate were all higher in the test group than in the control group.Conclusion: 1.PYD can significantly improve the symptoms of kidney deficiency and blood stasis in patients and may improve the embryo implantation rate and clinical pregnancy rate in frozen-thawed embryo transfer patients with long-term use.2.Herbs to tonify the kidneys and invigorate blood improve pregnancy outcomes in patients with thin endometrium by improving endometrial blood flow,in addition to possible improvements in endometrial thickness and volume.
