Objective:Based on the integrated database of clinical research,to retrospectively study the real world clinical prescription rules of TCM in the diagnosis and treatment of type 2 diabetic peripheral neuropathy.Methods:In this project,the clinical data of inpatients in the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2019 to September 2021 were collected by the integrated information system of clinical research,computer data mining and artificial intelligence,and the database of diabetic peripheral neuropathy was constructed.To study the distribution of drugs and syndrome types,drug-syndrome correspondence and core prescription of type 2 diabetic peripheral neuropathy.Results:(1)205 cases were initially included in this study,and 164 cases and 164 prescriptions were finally included.Data mining showed that the proportion of Yin deficiency and blood stasis in DPN was the highest,and the proportion of dampness and heat blocking collateral was the lowest.A total of 250 Chinese herbs were used in DPN prescriptions,with a cumulative frequency of 2566 times.The top 10 drugs in the frequency of use were Astragalus membranaceus,Angelica sinensis,cassia twig,Poria cocos,Ligusticum chuanxiong,Radix paeoniae,Achyranthus bidentata,Radix Ophiopogonis,Salvia miltiorrhiza and Rehmannia glutinosa.In the high-frequency drug dose statistics,the maximum dose of Astragalus membranaceus was 100 g,and the maximum dose of achyranthe bidentata was50 g.Among the 250 drugs,drugs for promoting blood circulation and removing blood stasis were the most frequently used,with frequency of427 times.(2)Angelica sinensis and Astragalus membranaceus were the top 10 drug pairs in frequency of high frequency drug compatibility.Angelica,cassia twig;Cassia twig,astragalus membranaceus;Astragalus and red peony root;Osmanthus twig,ox knee;Radix Ophiopogon,Astragalus membranaceus;Salvia miltiorrhiza and Astragalus;Astragalus membranaceus and Ligusticum chuanxiong;Rehmannia and Astragalus membranaceus;Pueraria root,astragalus membranaceus.After screening the drug-symptom relationship with the mutual information value ≧0.02,a total of 71 traditional Chinese medicines,45 special symptoms and 92 drug-symptom combinations were obtained.(3)Through the extraction of core TCM network,11 TCM combinations including radix ophiopogonis,radix rehmanniae,ligusticum chuanxiong,Angelica,radix paeoniae,Poria cocos,Astragalus membranaceus,cassia twig,salvia miltiorrhiza,Achyranthes bidentata and radix puerariae were obtained.Conclusion:The most common drug compatibility is the compatibility of tonifying drugs and activating blood drugs.Qi can produce blood,blood can carry qi,and tong tonifying is applied.The results of drug-symptom relationship show that in the real world,patients’ symptoms are complicated,and corresponding drug compatibility is often given according to the overall syndrome differentiation of patients.The core drug combination was identified by clinicians and was consistent with the prescription for invigorating spleen and reducing thirst,Huangqi guizhi Wuwu Decoction and Buyang Huanwu Decoction prepared by our hospital.The basic pathogenesis of DPN was based on spleen deficiency and blood stasis and collateral-blocking.The changes of core prescriptions based on core prescriptions have clinical significance.
