| Objective: To observe the clinical efficacy of warm needling combined with skin acupuncture in the treatment of temporomandibular joint disorders(TMD)syndrome of blood stasis block and,to preliminarily explore its therapeutic rationale,as to provide a kind of clinical treatment of TMD syndrome of blood stasis block a more convenient way.Methods: In this study,60 patients with temporomandibular joint disorders syndrome of blood stasis block were randomly divided into a treatment group and a control group,with 30 cases in each group.Both groups were treated with Xiaguan(ST7),Jiache(ST6),Tinggong(SI19),Ashi point,double Hegu(LI4),double Zusanli(ST36)and double Xuehai(SP10).Quanliao(SI18)and Shangguan(GB3)were used for patients with popping on this basis.In the control group was only treated with routine acupuncture,while the treatment group was treated with warm needling on Xiaguan(ST7),both sides of Zusanli(ST36)and Xuehai(SP10)after routine acupuncture,three strong moxibustion per point,and trapping afflicted part with skin acupuncture after warm needling.Both groups were treated once every other day,3 times a week as a course of treatment,for four consecutive courses.The changes of Fricton index score,Maximum active mouth opening,SF-MPQ score and TCM symptom score before and after treatment were observed in both groups,and the clinical efficacy was evaluated.Results: 1.Intra-group comparison: The scores of both groups were significantly reduced after treatment,with significant differences compared with those before treatment(P<0.05).2.Inter-group comparison: In terms of clinical efficacy,the total effective rate of the treatment group was 93.33%,and the total effective rate of the control group was 85.71%,with statistically significant differences between the two groups(P<0.05).In terms of Western medical observation indexes,the treatment group showed better improvement than the control group in MMO,muscle palpation(MP),palpation index(PI),craniomandibular index(CMI)and SF-MPQ score,and the differences were statistically significant(P<0.05),and the differences in the scores of the joint noise(JN),joint palpation(JP),mandibular movement(MM),dysfunction index(DI)were not statistically significant(P>0.05).In terms of TCM symptoms,the treatment group showed better improvement than the control group in the total syndromes scores,persistent local pain,aggravation of pain at opening,refusal to press on the affected area and easy fatigue,and the difference was statistically significant(P<0.05),while the differences in the Lip nail bule and purple was not statistically significant(P>0.05).Conclusion: Both warm needling combined with skin acupuncture and routine acupuncture have good clinical efficacy in the treatment of TMD syndrome of blood stasis block.Warm needling combined with skin acupuncture had an improvement effect on all TCM symptoms and was significantly better than routine acupuncture in terms of MMO,muscle palpation(MP),palpation index(PI),craniomandibular index(CMI),SF-MPQ score,total TCM symptom score and clinical efficacy,which is worthy of clinical promotion. |
