Effect Of Qingda Granule On Angiotensin ⅱ In Patients With Grade 1 Hypertension

Objective: To observe the effect of Qingda granule on Ang ⅱin the treatment of grade 1 hypertension patients with elevated Angiotensin ⅱ(Ang ⅱ).To investigate the mechanism of Qingda granule on renin angiotensin-aldosterone system(RAAS).Methods: Our hospital participated in this study as a sub-center from the national multi-center,large-sample trial,which was designed as a randomized,double-blind,double-simulated,parallel-controlled,non-inferior clinical study.Forty grade 1hypertension patients with elevated Ang ⅱ syndrome of hyperactivity of liver Yang were included as research subjects and randomly divided into control group(n=17)and experimental group(n=23).The control group was given 80 mg q D of Valsar tan capsule and 5g Bid of Qingda granule simulant,while the experimental group was given 5g Bid of Qingda granule and 80 mg Qd of Valsartan capsule simulant.The treatment course was 4 weeks,followed up for 4 weeks after drug withdrawal.The clinical blood pressure,24-hour ambulatric blood pressure and Ang ⅱ were used as outcome indexes4 weeks after treatment and 4 weeks after drug withdrawal.Results:(1)Before treatment,baseline characteristics were comparable between groups(ALL P > 0.05).(2)After 4 weeks of treatment,the office blood pressure and 24 h am BUlatory blood pressure of the two groups decreased significantly c ompared with before treatment(P < 0.01 or P < 0.05),but there was no statistical difference between the two groups(P > 0.05);4 weeks after drug withdrawal,blood pressure in 2 groups returned to baseline blood pressure level,with no statistical differ ence between the two groups(P > 0.05).(3)After 4 weeks of treatment,Ang ⅱ in both groups decreased significantly compared with before treatment,and the results were statistically significant(P <0.05),but there was no statistical difference between the two groups(P > 0.05);After 4 weeks of drug withdrawal,compared with4 weeks of treatment,Ang ⅱ in both groups returned to the baseline level,with significant statistical significance(P <0.05),while there was no significant statistical differenc e between groups(P > 0.05).(5)During the experiment,no obvious adverse events were observed in both groups.Conclusion:Compared with Valsartan capsules,the non-inferiority comparison of Qingda Granules alone in lowering blood pressure and Ang II for 4 weeks was established for patients with grade 1 liver-yang hyperactivity syndrome with elevated Ang II.The main antihypertensive mechanism of Qingda granule is that there may be a specific target similar to Valsartan that can rapidly reduce Ang II,so it is speculated that Qingda granule may inhibit RAAS by antagonizing angiotensin II type 1 receptor(AT1R).The mechanism of lowering blood pressure is partly similar to that of angiotensin II receptor blockers(ARBs),which can lower blood pressure by abou t 10/5mm Hg.And Qingda Granules are safe and have no obvious adverse reactions.
