Subject : To study the dif f e re nce o f Accommodative Microfluctuations and Accommodative Function be twe e n myo pic and e mme tr o pic c hildre n and the c or re latio n be twe e n Accommodative Microfluctuations and Accommodative Function,e ye par ame ter s.To explore the role of accommodative microfluctuations in the on-set and development of myopia.Method:PartⅠ: A cross-sectional study was conducted on 276 children and adolescents aged from 8 to 14 years old.According to the refraction after mydriatic optometry,subjects were divided into 2 group:emmetropia group;myopia group.the subjects were examined by SER,IOP,AL,ACD,Km and accommodative function.Measures of accommodative response and microfluctuations were obtained for 30 s using a Grand Seiko WAM-5500 infrared autorefractor while subjects read binocularly through Maltese Cross at 33 cm respectively.The differences of AMFs,LA,AF,AMP and other eye parameters between the two groups were analyzed.At the same time,the correlation between AMFs and LA,AF,AMP,other eye parameters in myopia group was analyzed.PartⅡ:According to the inclusion and exclusion criteria of the subjects,60 myopic children aged 8-14 years were selected to be treated with acupuncture.Select the main acupoints:Cuanzhu,Yuyao,Sizhukong,Sibai,Taiyang;matching acupoints: Hegu,Zusanli,Guangming.The needles were retained at all acupoints for 30 minutes.After taking up the needles,the relevant eye examination was carried out again.The patients were treated twice a week and one month as a course of treatment.The AF,AMP,LA and AMFs were measured before acupuncture treatment and 30 min,1 month after treatment,and the differences among the three periods were compared.Results: AMFs and LA in myopia group were higher than those in emmetropia group(P< 0.001),AF and AMP in myopia group were lower than those in emmetropia group,there was significant correlation between LA,AF and AMFs in myopia group,but there was no significant correlation between age,mean corneal curvature,ACD,AL,IOP,SER,AMP and AMFs.UCVA and AF at 1 week and 1 month after acupuncture treatment were significantly higher than those before acupuncture treatment,and the difference was statistically significant(P < 0.001),but there was no significant difference in UCVA and AF after acupuncture treatment with 30 min.The AMP of 30 min,1 week and 1 month after acupuncture treatment was significantly higher than that before acupuncture treatment,and the difference was statistically significant(P < 0.01).AMP will continue to increase with the extension of treatment time.The HFC of subjects at 1 week and 1 month after acupuncture treatment was lower than that before acupuncture treatment and after acupuncture treatment,and the difference was statistically significant(P < 0.001),but there was no significant difference in HFC between 1 month and 1 week acupuncture treatment.The LA of 30 min,1 week and 1 month after acupuncture treatment was lower than that before acupuncture treatment(P < 0 01),but there was no significant difference in LA between 1 week and 1 month after acupuncture treatment.Conclusion: Compared with emmetropic children,myopic children had larger AMFs,LA and smaller AMP,AF.Positive correlation is found among accommodative lag and AMFs,negative correlation is found among AF and AMFs in myopic children.there was no significant correlation between Km,ACD,IOP,AL,SER,AMP and AMFs in myopic children.Acupuncture can improve UCVA,HFC,LA,AF and AMP in myopic children,but has no significant effect on accommodative variability and LFC.
