Effect Of Yishen Gudi Decoction On Hyperhomocysteine And Renal Tubular Function In Early Hypertensive Renal Damage

Objective:To observe the effect of Yishen Gudi Decoction on hyperhomocysteinemia and renal tubular injury in early renal damage in hypertension,and explore the mechanism of treating this disease with integrated traditional Chinese medicine and modern medicine,so as to provide new ideas for clinical treatment of early stage of hypertension renal injury.Methods:Collect 78 hypertensive patients with early renal damage and hyperhomocysteinemia in the outpatient and ward of Department of Nephrology,Affiliated Hospital of Shandong University of Traditional Chinese Medicine,which meet the selection criteria,and randomly assign them to the experimental group and the control group by random number table method.The control group was given folic acid tablets on the basic treatment of modern medicine,while the experimental group was given Yishen Gudi Decoction on the basic treatment of modern medicine.After 12 weeks of treatment,the changes of systolic blood pressure,diastolic blood pressure,serum homocysteine,urinary α 1-microglobulin,urinary β2-microglobulin,urinary N-acetyl-β-D-glucosaminidase and TCM symptom scores,and the changes of safety indexes were observed and statistically analyzed.Results:(1)Before treatment,there was no significant difference in baseline and related indicators between two groups(P>0.05).(2)Before and after treatment,the systolic blood pressure and diastolic blood pressure of two groups were compared within group,and differences were statistically significan(P<0.01),which were lower than those before treatment.After treatment,the systolic blood pressure and diastolic blood pressure between the two groups were significantly different(P<0.01).(3)Before and after treatment,there were significant differences in serum homocysteine,urinary microalbumin,urinary α1-microglobulin,urinary β2-microglobulin,urinary N-acetyl-β-D-glucosaminidase and TCM symptom scores between within group.After treatment,there were significant differences in serum homocysteine,urinary N-acetyl-β-D-glucosaminidase and TCM symptom scores between two groups(P<0.01),and there were significant differences in urinary α1-microglobulin and urinary β2-microglobulin(P<0.05).(4)The correlation analysis,homocysteine with α1-microglobulin,β2-microglobulin and N-acetyl-β-D-glucosaminidase,showed that homocysteine had positive rank correlation with urinaryα1-microglobulin,urinary β 2-microglobulin and N-acetyl-β-D-glucosaminidase(P<0.01).Conclusion:Yishen Gudi decoction can effectively improve the clinical symptoms of TCM,improve the quality of life of patients,reduce the level of serum homocysteine,reduce the risk factors of renal damage,restore renal tubular function and improve the prognosis.and the level of serum homocysteine is closely related to the function of renal tubules.
