| Objective:Styloid process syndrome(SPS),also known as Eagle syndrome,has various clinical manifestations and is often misdiagnosed.Therefore,this study conducted a retrospective analysis method to analyze its general clinical data and surgical treatment effects.The purpose of this study is to improve the correct rate of the first diagnosis of Eagle syndrome,improve the quality of life of patients after surgery,and provide clinical reference for the diagnosis and treatment of Eagle syndrome by oral and maxillofacial surgeons.Methods:A total of 157 patients with Eagle syndrome who underwent styloid process amputation in the First Affiliated Hospital of Zhengzhou University from June 2011 to June 2021 were selected as the research objects.The general clinical data of the patients were retrospectively analyzed,including gender,age,chief complaint,length of styloid process,surgical treatment methods and efficacy,postoperative pharyngeal foreign body sensation,and VAS score.SPSS software was used for statistical analysis,and chi-square test was used to analyze the correlation between styloid process excess and the actual prevalence;Chi-square test was used to analyze the therapeutic effect of different surgical approaches on styloid process syndrome and postoperative pharyngeal foreign body sensation.influence.Results:1.Clinical manifestations:Among the 157 patients,there were 58 males(36.94%)and 99 females(63.06%),most of whom were females,aged 28-84(52.2±12.0)years old.There were 38 patients(24.20%)with bilateral disease and 119 patients(75.80%)with unilateral disease,and unilateral disease was the most common.Among the 157 patients,126(80.25%)complained that there wa s no obvious incentive,and 31(19.75%)complained that there was a clear inc entive.Most of the patients had no obvious incentive.Among the 157 patients,40 patients(25.48%)had a history of less than 3 misdiagnoses,and 117 pati ents(74.52%)had a history of more than 3 misdiagnoses.Misdiagnosed as chr onic pharyngitis in 68 cases(43.31%),tonsillitis in 30 cases(19.11%),glossop haryngeal neuralgia in 23 cases(14.65%)and other diseases.2.Diagnosis and treatment of stomatology:easily misdiagnosed as temporomandibular joint disorder in 14 cases(24.56%),glossopharyngeal neuralgia in 11 cases(19.30%),cervical spondylosis in 9 cases(15.79%),and odontogenic pain in 6 cases(10.53%),3 cases of trigeminal neuralgia(5.26%),2 cases of burning mouth disease(3.51%)and other diseases.3.Imaging manifestations:CT imaging examination showed that 157 patients(100%)were in line with styloid process,140 patients(89.17%)had bilateral styloid process,and 38 patients(24.20%)were actually bilateral.There was a statistical difference between the imaging manifestations of excessive styloid process and the actual disease(P<0.05).The imaging manifestations did not match the actual disease situation,and clinical manifestations may not appear when the styloid process was too long.4.Pathological manifestations:The pathological manifestations are non-specific,showing general bone tissue,only bone tissue,bone marrow,mature lamellar bone,cartilage components or fibrous tissue can be seen.5.Efficacy and evaluation:There were 134 cases of intraoral approach,13 cases were ineffective(9.7%),and the remaining 121 cases were effective(90.3%),of which 67 cases(50.00%)were cured and 54 cases(40.30%)improved;external cervical approach There were 23 cases in total,2 cases were invalid(8.70%),and the remaining 21 cases were effective(91.30%),of which 17 cases(73.91%)were cured and 4 cases(17.39%)improved.The preoperative VAS score was(5.30±1.64),and the postoperative VAS score was(1.58±1.66).The paired sample t test was performed on the preoperative and postoperative VAS scores,and there was a statistically significant difference(P<0.05).The surgical treatment was effective;The χ2 test was performed on the therapeutic effect of the approach and the external cervical approach,and there was no statistical difference(P>0.05).There was no difference in the therapeutic effect between the intraoral approach and the external cervical approach.6.Postoperative pharyngeal foreign body sensation:There were 43 cases(32.09%)of foreign body sensation in the pharynx after the intraoral approach,and 2 cases(8.7%)of the foreign body sensation in the pharynx after the external cervical approach.The χ2 test was performed on the two,and there was a statistical difference(P<0.05),there is obvious pharyngeal foreign body sensation after intraoral approach.Conclusion:1.Styloid process syndrome and oral and maxillofacial pain diseases have cross symptoms,which is the reason for easy misdiagnosis;when patients have symptoms such as pharyngeal pain,the possibility of styloid process syndrome should be considered,and pharyngeal palpation should be performed.Diagnosis and CT of the styloid process.2.The current surgical methods mostly use the intraoral approach to achieve aesthetic results,but the incidence of postoperative pharyngeal foreign body sensation is low in the external cervical approach.the therapeutic effect. |
