Background:Homeostasis Model Assessment of Insulin Resistance(HOMA-IR)is an important indicator for the evaluation of insulin resistance(IR),and has been widely used in the study of insulin-related endocrine and metabolic diseases by domestic and foreign scholars.IR is defined as a reduction in the relative insensitivity or responsiveness of peripheral target organs and target tissues to biomodulation stimulated by normal levels of insulin.According to the World Health Organization,the cut-off point for obesity or overweight in Asian populations is defined as a body mass index(BMI)>25 kg/m2.Studies have shown that obesity and IR are not only independent risk factors for infertility,but also the main clinical manifestations of polycystic ovary syndrome(PCOS).These infertile patients may require in vitro fertilization-embryo transfer(IVF-ET)technical treatment.In addition,studies have shown that obesity can cause IR,aggravate the degree of IR,cause metabolic abnormalities in patients,and ultimately lead to the occurrence of adverse outcomes such as female ovulation and poor embryo quality.However,not all patients with infertility have signs of obesity,and some patients only have insulin resistance.Therefore,the purpose of this study was to observe the effect of IR on pregnancy outcomes in normal-weight infertile patients..Objective:To determine the cutoff values of HOMA-IR for predicting clinical pregnancy rate in normal weight patients during their first IVF.Materials and methods:Data from 241 normal-weight women aged 21-40 years,18.5 kg/m2<BMI<25 kg/m2 who underwent IVF-ET for the first time between June 2020 and December 2020 were retrospectively analyzed.The association between HOMA-IR during IVF and clinical pregnancy rates was assessedResults:The higher the HOMA-IR,the worse the IVF pregnancy outcome.Considering that IR is a common complication of PCOS,we grouped patients according to whether they had PCOS or not.Results of IVF assisted pregnancy in infertile women In the non-PCOS group,the clinical pregnancy rate was significantly decreased at the HOMA-IR cut-off point of 2.57(OR 0.018)(OR=0.468,95%CI:,0.240-0.913;p<0.05).Conclusion:The results of this study suggest that,with or without PCOS,higher HOMA-IR adversely affects lVF outcomes in infertile women.HOMA-IR can be used as a predictor of IVF clinical pregnancy rate.
