Objective To compare the uterine volume,cervix length and uterine artery blood flow in patients with polycystic ovary syndrome(PCOS)with normal population.To evaluate the clinical value of using uterine parameters to predict pregnancy outcome in patients with PCOS.Methods Prospectively included 203 patients with PCOS who were admitted to the Reproductive Center of our hospital from January 2018 to January 2019 as the experimental group.50 patients in the normal population who were treated in our hospital for the same period were selected as the control group.On the third day of menstruation(the amenorrhea,the third day after withdrawal of progesterone from the blood),vaginal color Doppler ultrasound(TV-CDS)was used to measure uterine volume,cervical length and uterine artery blood flow parameters,and venous blood was collected at 5 ml.For the detection of sex hormones.5 ml of venous blood was collected during non-menstrual period,and serum was used for fasting blood glucose(FBG)and fasting insulin(FINS).The relationship between the experimental group and the control group,the sex hormones,metabolic indicators and uterine parameters were explored.The relationship between uterine parameters and age,BMI,hyperandrogenism,and insulin resistance were explored in the experimental group.The experimental group was divided into the promotion cycle and the IVF-ET cycle according to the treatment plan to investigate the relationship between uterine parameters and the clinical pregnancy in the promotion cycle and IVF-ET cycle.Results1.Comparison of uterine parameter measurement between experimental group andcontrol groupThe uterus volume of the experimental group was 36.9±1.42cm3,the length of the cervix was 2.91±0.37 cm,and the S/D value of the uterine artery was 6.80(4.85,7.94),which was smaller than the uterus volume of the control group by 48.15±1.64cm3,and the length of the cervix was 3.26±0.35 cm and S/D.The value was 9.90(3.26,7.65)and the difference was statistically significant.The RI value of uterine artery in the experimental group was 0.87(0.79,0.88),which was significantly higher than the RI value of the control group by 0.85(0.73,0.89).The difference was statistically significant.2.Comparison of uterine parameter and age,BMI,androgen and insulin resistance in the experimental group(1): In the experimental group,according to age 20 ≤ age <25 years old,group 1,25≤ age < 30 years old,divided into group 2,≤ 30 years old,divided into group 3.The uterine volume of group 1 was 31.87±11.376cm3,which was smaller than the uterine volume of group 2 of 38.44±13.83cm3,and the uterus volume of group 3 was41.72±16.74cm3.The difference was statistically significant.There is no difference in the length of the cervix.(2): The uterus volume of the obese group(BMI≥28kg/m2)in the experimental group was 38.51±16.29 cm3 and the length of the cervix was2.94±0.33 cm,and the uterus volume of overweight(BMI≥24kg/m2)was37.93±13.78cm3 and the length of the cervix was 2.88±0.32.There was no significant difference in cm between the uterus volume of the normal weight group of37.14±13.713 and the cervical length of 2.92±0.37 cm.(3): In the experiment,the uterus volume of the high androgen group(T≥1.8mmol/L)was 34.65±12.16cm3,the length of the cervix was 2.83±0.32 cm,the uterus volume of the normal androgen group was 39.66±15.17cm3,and the length of the cervix was 2.97±0.37 cm.The difference is statistically significant.(4): The uterus volume of the insulin resistance group(HOMA-IR≥2.69)in the experimental group was 37.98±1.47cm3,the length of the cervix was 2.92±0.37 cm,and the uterus volume of the non-insulin resistance group was 37.01±1.35cm3,and the length of the cervix was 2.91±0.35 cm.Thedifference was not statistically significant.3.Relationship between uterine parameters and clinical pregnancy in the experimental groupIn the IVF-ET cycle,the uterus volume of the pregnant group was 39.82±1.79cm3 and the length of the cervix was 3.17±0.31 cm larger than that of the non-pregnant group.The volume of the uterus was 34.97±1.07cm3 and the length of the cervix was2.94±0.35 cm,but it was not statistically significant.During the promotion cycle,the uterus volume of the pregnant group was38.29±12.47cm3,which was larger than that of the non-pregnant group,33.21±12.30cm3,but the difference was not statistically significant.The length of the cervix in the pregnant group was 3.00±0.35 cm,which was greater than that in the non-pregnant group,2.80±0.42 cm.The difference was statistically significant.Conclusion The uterine volume of the PCOS patients,the length of the cervix is smaller than the normal population,smaller uterine volumes are associated with lower age and high androgen,and shorter cervical lengths are associated with high androgen.Measurement of uterine volume and cervical length of PCOS patients by TV-CDS has certain clinical value for predicting clinical pregnancy.
