Objective:To explore the surgical method and clinical effect of nasolabial flap in the repair of nasal defects.Method:Reviewing the case data of patients with nasal defects treated in the nasal plastic surgery and reconstruction center of the plastic surgery hospital of the Chinese Academy of Medical Sciences from April 2016 to December 2021,14 patients were selected to repair nasal defects with nasolabial groove skin flap.During the operation,the nasolabial groove skin flap was designed according to the location and size of the defect,and the shape of nasal alar,nasal tip,nasal column and nasal dorsum were reconstructed.The pedicle was broken in phase II 3-12 months after the operation,and then the skin flap was usually repaired for many times Skin flap thinning and scar resection to improve the appearance.According to the defect location and scope,postoperative complications,the number and time span of multi-stage surgery,postoperative aesthetic effect,patient satisfaction and so on.Results:All 14 patients were followed up.The follow-up time was 4~24 months after operation.All the flaps survived,and one patient had venous congestion after operation;All patients underwent multi-stage surgery;After the first operation,the appearance of the nose is relatively poor,but it can still show a good appearance repair effect after multiple operations;In order to achieve the final effect,an average of 3.14 operations were needed,and the average time span was 14.57 months;The patients who were followed up after multi-stage surgery were highly satisfied with the color and appearance of the nose.Their dissatisfaction mainly focused on the problems such as the number of operations,the long time span of the whole process,the high cost and so on.Conclusion:Nasolabial groove flap is close to the nose and has no vascular restrictions.It is a good surgical method for repairing nasal alar defects because of its flexibility and adaptability.However,the pedicle is hypertrophic,the flap is thick,and the flap transfer process is easy to lead to the formation of scar and deformity.In addition to the stage Ⅱ flap pedicle breaking operation,it also needs multi-stage trimming to achieve better aesthetic effect.Multiple trimming operations,long treatment span and high cost also bring some inconvenience to patients.
