School doctors and nurses refer to school health personnel with health technical qualifications,who play a vital role in maintaining the growth and development of children and adolescents.Under the new situation,the job responsibilities and work content of school doctors and nurses are changing.There are some problems such as imperfect promotion mechanism,low staffing rate,and marginalized roles,causing stress and anxiety of school doctors and nurses.Their health status faces challenges.Based on the background of "Healthy China",health-related quality of life has became a significance key point to measure people health.To explore the current situation and influencing factors of the health-related quality of life of school doctors,and to investigate the health-related quality of life of school doctors from the perspective of self-reporting,so as to stabilize the team construction of school doctors,improve the working capacity of school health service and maintain the healthy growth of children and adolescents.This study adopted the method of multistage stratified sampling and selected primary and secondary school doctors in various cities in Shandong Province during June to July 2021 according to online survey.The EuroQoL five-dimension questionnaire(EQ-5D-5L)and the World Health Organization Five-item Well-Being Index(WHO-5)were used to evaluate the health-related quality of life of the sample population,and to explore the internal relationship between the two scales.Sociodemographic characteristics and occupational information,using the self-made questionnaire to collect data.The Minnesota Job Satisfaction Scale(MSQ)to describe the job satisfaction of school medicine in primary and secondary schools.The differences in the quality of life scores of the sample population under different characteristics were analyzed using non-parametric tests,Tobit regression and multiple linear regression were used in regression analysis.502 primary and secondary school doctors and nurses were included in the analysis,with an average age of 40.9±8.9 years old,mainly women(70.5%),and more than half of the full-time career-based the sample population;the average score of overall job satisfaction was 4.03±0.59 points,among the satisfaction rate of job recognition,salary and promotion opportunities were lower.The health utility measured by the EQ-5D-5L scale was 0.963 ±0.076,The proportion of "no problem" in mobility,self-care and daily activities was more than 95%,and the proportion of pain/discomfort and anxiety/depression were 76.89%and 77.68%,respectively.The mental health score measured by the WHO-5 scale was 16.11±6.90,and 27.1%of the school doctors and nurses experienced mental health damage.The two scales showed some complementarity when measuring the health-related quality of life of school doctors at the same time.The non-parametric test results show that different post treatment,years of medical work,school type and boarding conditions had statistical differences on health-related quality of life scores of school doctors.The results of regression analysis showed that post treatment,and job satisfaction were important factors affecting health-related quality of life of school doctors in shandong province.This study found that the medical staffs of primary and secondary schools in Shandong province are mainly middle-aged and young women,and their overall job satisfaction is above medium level,but their external job satisfaction in terms of job recognition,salary and promotion opportunities are low.The general health status of primary and secondary school doctors and nurses is good,but there are certain problems in pain/discomfort,anxiety/depression.We should focus on the health-related quality of life of the sample population with low job satisfaction,career establishment,work location in high school,boarding system,and working years>15 years.Based on the above conclusions,relevant policy suggestions are put forward:pay attention to the construction of the sample population and improve their social status;pay attention to the psychological health of the sample population and strengthen the counseling of bad emotions;pay attention to the job satisfaction of the sample population and improve the physical and mental health of school doctors. |
