Objective:By analyzing the influence of intervention program of Orem self-care theory assisted education system on self-care ability,quality of life and gastrointestinal symptoms of elderly patients with chronic atrophic gastritis,the application effect of Orem self-care theory assisted education nursing was determined.It provides a reference for hospitals and communities to establish the Orem self-care theory assistance-education nursing system for elderly patients with chronic atrophic gastritis.Method:This study adopts an unequal control group design similar to experimental studies.From February to May 2021,182 elderly patients with chronic atrophic gastritis in the outpatient department of a third grade A hospital in W City,Shandong Province were selected as the research objects.The convenience sampling method was used.There were 91 cases in the control group(routine nursing)and 91 cases in the intervention group(routine nursing+the auxiliary education intervention scheme of Orem self-care theory).The intervention time was 6 months.The intervention methods included wechat group pushing health information,wechat answering questions,telephone follow-up and follow-up education.Through the general information questionnaire,self-care ability measurement scale(ESCA),simple health survey questionnaire(SF-36).SPSS26.0 software was used to analyze the survey data.The statistical data were described by frequency and composition ratio,and chi-square test(χ~2)was used.The measurement data followed normal distribution and were described by mean and standard deviation.The intervention effect was evaluated by independent sample t-test,and the difference was statistically significant with P<0.05.Test levelα=0.05.Result:(1)General data of subjects:among the 182 patients who completed this study,105 were male,accounting for 57.7%of the total sample size;Marital status:there are 130 spouses at most,accounting for 71.4%;Living style:148 cases lived with their families,accounting for 81.3%;There were 156 cases of Helicobacter pylori infection,accounting for 85.7%;109 cases had irregular eating habits,accounting for 59.9%.(2)Homogeneity test of the two groups before intervention:In this study,there was no statistical significance in the comparison of general demographic data,disease data,self-care ability,quality of life and gastrointestinal symptoms(P>0.05),indicating comparability.(3)Comparison of self-care ability between the two groups before and after intervention:There was no significant difference in the total score and dimension of the control group before and after intervention(P>0.05).The total score and dimensions of self-nursing ability in the intervention group after intervention were higher than those before intervention,with statistical significance(P<0.05).(4)Comparison of the quality of life between the two groups before and after the intervention:In the control group of this study,the total mental health score after intervention(255.71±28.15)was higher than that before intervention(252.03±26.34),the difference was statistically significant(P<0.05),and the rest had no statistical difference(P>0.05).There was no statistical difference except for body pain in the intervention group(P>0.05),there were statistically significant differences in other dimensions scores and total scores(P<0.05).The mental health scores of the control group and the intervention group before and after intervention were compared between the groups,and the results showed that the difference score of the intervention group was higher than that of the control group,with statistical significance(P<0.05).(5)Comparison of gastrointestinal symptoms between the two groups before and after intervention:In this study,the total score and dimensions of the control group before and after intervention showed no statistical significance(P>0.05);there were statistically significant differences in the intervention groups(P<0.05).Conclusion:(1)The assistive education system intervention based on Orem self-care theory can improve the self-care ability of elderly patients with chronic atrophic gastritis;(2)The aid-education system intervention based on Orem self-care theory can improve the quality of life of elderly patients with chronic atrophic gastritis;(3)The aid-education system intervention based on Orem self-care theory can improve the gastrointestinal symptoms of elderly patients with chronic atrophic gastritis;(4)When carrying out the auxiliary-educational intervention program of Orem self-care theory for elderly patients with chronic atrophic gastritis,according to the characteristics and receptivity of chronic atrophic gastritis in elderly patients,the network mobile platform should be fully utilized to understand the needs of patients in a timely manner,and Targeted Orem auxiliary-educational nursing should be carried out to ensure the effectiveness of intervention. |
